夹脊穴电针降低带状疱疹神经痛患者的疼痛

The Relief of Pain in Patients with Herpes Zoster Neuralgia by Using Jiaji Electroacupuncture

为了探究夹脊穴电针治疗带状疱疹神经痛的疗效及其可能的分子机制，本研究选取了80例带状疱疹神经痛患者，并随机将其分为观察组和对照组，每组各40例。观察组行夹脊穴电针为主的针刺治疗；对照组给予常规西药治疗，两组均以10d为一个疗程。通过观察治疗前后两组的镇痛效果，抑郁状态改善效果及综合疗效，并进一步检测治疗前后IL-6、IL-8及TNF-α等炎性细胞因子水平，本研究发现治疗前两组的视觉模拟评分（VAS评分）、抑郁量表评分及血清IL-6、IL-8、TNF-α水平比较无统计学差异（p〉0．05）；治疗后两组的VAS评分、抑郁量表评分及血清IL-6、IL-8、TNF—α水平较治疗前显著降低，且治疗后观察组的VAS评分、抑郁量表评分及血清IL-6、IL-8、TNF-α水平显著低于对照组（P〈0．05）；观察组的综合临床疗效显著优于对照组（p〈0.05）。本研究表明，夹脊穴电针可以显著减轻带状疱疹患者的疼痛程度，并能改善抑郁程度，综合疗效优于单纯西药治疗，其机制可能与减轻炎性反应有关。我们的研究为夹脊穴电针在带状疱疹神经痛的临床治疗和分子机制的研究提供了一定的依据。

In order to explore the therapeutic effect and possible molecular mechanism in the treatment of herpes zoster neuralgia using Jiaji electroacupuncture, we selected 80 patients with herpes zoster neuralgia and randomly divided them into observation group and control group, with 40 cases in each group. The observation group was given acupuncture treatment with Jiaji electroacupuncture as the main part; the control group was treated with conventional western medicine, and both groups were treated with 10 days as a course of treatment. By observing the analgesic effect, the improvement effect of depression and the comprehensive curative effect of two groups before and after treatment, and further detection of the levels of inflammatory cytokines such as IL-6, IL-8 and TNF-α before and after treatment, we found that there was no significant difference between the two groups in visual analogue scale （VAS）, depression rating scale and the levels of serum IL-6, IL-8 and TNF-α before treatment （p〉0.05）; After treatment, the VAS scores, depression rating scale scores and the levels of serum IL-6, IL-8 and TNF-α in the two groups were significantly lower than those before treatment, as well as after treatment, the VAS scores, depression rating scale scores and the levels of serum IL-6, IL-8 and TNF-α in the observation group were significantly lower than those in the control group; the comprehensive clinical effect of the observation group was significantly better than that of the control group （p 〈0.05）. The research indicated that Jiaji electroacupuncture could significantly reduce the pain of patients with herpes zoster and improve the depression degree. The comprehensive curative effect was better than that of simple western medicine. The mechanismmight be related to the reduction of inflammatory response. Our study might provide a basis for the study of the clinical treatment and molecular mechanism of herpes zoster neuralgia by Jiaji electroacupuncture.
