基于灰色关联理论的临床路径管理模式应用效果

Analysis of the Application Effects of Clinical Pathway Management Model Based on Grey Correlation Theory

目的分析探讨基于灰色关联理论的临床路径管理模式的应用效果。方法以临床路径管理病种平均住院费用作为参考序列,入径率、变异率、完成率、平均住院日、药费比、治愈好转率、再入院率作为比较序列,计算灰色关联系数,量化关联程度,明确管理重点,进行有针对性的、动态的周期性管理。结果通过运用基于灰色关联理论的临床路径管理模式,从2014至2016年,医院管理人员未增加,临床管理病种数由46个上升到111个,入径率由74.77%上升到90.04%,完成路径例数占出院人数比例由5.43%上升到50.79%,平均住院日由9.47天下降到6.88天,药占比由36.16%下降到32.89%。结论基于灰色关联理论的临床路径管理模式能够量化管理目标,突出管理干预重点,节约管理人力资源,提高管理效率,保证管理质量,有利于临床路径在各医院的推广。

Objective To explore the application effects of clinical pathway management model based on Grey Correlation Theory. Methods With the average hospitalization expenses of clinical pathway disease as the reference sequence and with the access rate, mutation rate, completion rate, average hospitalization days, drug proportion, cure and improvement rate, and readmission rate as the comparative sequence, we calculated the grey correlation coefficient, quantified the association degree, and clarified key in management, so as to conduct highly individualized and dynamic cyclical management. Results By applying the clinical pathway management model based on Grey Correlation Theory, from 2014 to 2016, the quantity of hospital administrative staff remained stable, the number of clinical management diseases increased from 46 to 111, the access ratio increased from 74. 77 % to 90.04% , the proportion of completion cases in hospital discharges increased from 5.43% to 50. 79 % , the average hospitalization days decreased from 9. 47 to 6. 88, and drug proportion decreased from 36. 1 6 % to 32. 8 9 % Conclusion Clinical pathway management model based on Grey Correlation Theory can quantify management objec-tives ,highlight the focus of management intervention, save human resources, improve management efficiency and ensure the management quality, which is conducive to the promotion of clinical pathway in hospitals.
