腋下小切口与后外侧切口手术对食管癌患者术后肺部感染及肺功能的影响

Effects of subaxillary small incision and posterolateral incision operation on postoperative pulmonary infection and lung function in patients with esophageal carcinoma

目的探讨腋下小切口与后外侧切口手术对食管癌患者术后肺部感染及肺功能的影响。方法选取在本院进行治疗的食管癌患者68例,按随机数表法分为两组各34例。对照组行后外侧切口开胸手术治疗,观察组行腋下小切口手术治疗,比较两组肺功能指标及并发症情况。结果术后1个月,观察组肺活量(VC)、用力肺活量(FVC)、最大通气量(MVV)、第1s用力呼吸容积(FEV1)明显高于对照组,差异有统计学意义(P<0.05);观察组肺不张及肺部感染发生率低于对照组,差异有统计学意义(P<0.05)。结论与后外侧切口手术对比,腋下小切口手术可减少食管癌手术对患者肺功能的影响,降低肺部感染等并发症发生率。

Objective To investigate the effect of subaxillary small incision and posterolateral incision operation on postoperative pulmonary infection and lung function in patients with esophageal carcinoma.Methods We selected 68 patients with esophageal cancer patients treated in our hospital who were randomly divided into two groups,34 cases in each group.The control group was treated with thoracotomy,and the observation group was treated with subaxillary small incision.The pulmonary function indexes and complications of the two groups were compared.ResultsOne months after surgery,the observation group of vital capacity（VC）,forced vital capacity（FVC）,maximal ventilatory volume（MVV）,forced expiratory volume in 1 s（FEV1）was significantly higher than of the control group,the difference was statistically significant（P〈 0.05）;the observation group atelectasis and pulmonary infection rate is lower than that of control group,the difference was statistically significant（P〈0.05）.Conclusion Compared with the posterolateral incision surgery,subaxillary small incision surgery can reduce the postoperative pulmonary function and reduce the incidence of complications such as lung infection.
