ICU肺动脉高压合并呼吸衰竭患者采用有创-无创序贯机械通气治疗的临床效果分析 被引量:9

摘要 目的探讨有创-无创序贯机械通气治疗ICU肺动脉高压合并呼吸衰竭患者的临床效果。方法选取本院ICU收治的92例肺动脉高压合并呼吸衰竭患者为研究对象,随机分为对照组与观察组各46例,对照组患者采用常规无创机械通气治疗,观察组患者采用有创-无创序贯机械通气治疗,比较两组患者治疗后的血气分析指标、血浆脑钠肽、并发症情况、机械通气治疗时间以及住院时间。结果观察组患者的机械通气时间、住院时间、呼吸机相关肺炎发生率、治疗后的pH值、PaO_2、PaCO_2、SaO_2与脑钠肽水平与对照组相比,差异均有统计学意义(P<0.05)。结论针对ICU肺动脉高压合并呼吸衰竭患者采用有创-无创序贯机械通气治疗可改善其血气分析指标,提高通气功能,也可缩短治疗时间,减少并发症的发生。 Objective To discuss the effect of invasive non-invasive sequential mechanical ventilation for pulmonary hypertension patients with respiratory failure in ICU.Methods A total of 92 cases of pulmonary hypertension complicated with respiratory failure in ICU of our hospital were randomly divided into the control group and the observation group,each with 46 cases.The patients in control group were treated with conventional non-invasive mechanical ventilation,and the patients in observation group were treated with invasive non-invasive sequential mechanical ventilation.The indexes of blood gas analysis,plasma brain natriuretic peptide,complications,mechanical ventilation,treatment time and hospitalization time were compared between the two groups.Results There was statistically significant difference between the two groups in the mechanical ventilation time,length of stay,the incidence of ventilator-associated pneumonia,the index of pH,PaO2,PaCO2,SaO2 and brain natriuretic peptide(P〈0.05).Conclusion The invasive non-invasive sequential mechanical ventilation can improve the blood gas analysis index of patients with pulmonary hypertension accompanied by respiratory failure in ICU.It can improve the ventilation function,shorten the time of treatment and avoid complications.
