三维可视化技术在肝门部胆管癌诊断与治疗中的转化应用 被引量:17

Translational application of three-dimensional visualization technology in the diagnosis and treatment of hilar cholangiocarcinoma

摘要 数字医学经历了从数字虚拟人技术到数字医学技术的转化,近10年来在临床医学各方面均有广泛应用,尤其是肝门部胆管癌,其因手术难度大,术前传统影像学(CT、MRI等)检查评估难以精准判断肿瘤横向及纵向侵犯情况。三维可视化技术在CT、MRI等检查获得器官和病变信息基础上,可获得更多、更清晰、更准确的数据;能直观、立体、准确地显示肿瘤侵犯周围重要血管的范围及血管变异情况,在肝门部胆管癌术前评估、手术规划、提高手术成功率、降低术后并发症发生率等方面发挥了重要作用,改变了“摸着石头过河”,实现了“看着石头过河”。数字医学技术在肝门部胆管癌的临床应用是转化医学的成功范例。 Digital medicine underwent the conversion from “digital human” to digital medical technology and has been widely used in all aspects of clinical medicine in recent 10 years, especially in hilar cholangiocarcinoma (HCCA). It is difficult to accurately evaluate the longitudinal and vertical tumor invasion by preoperative conventional imaging assessment (CT, MRI, etc.) due to the complexity of this operation. Three-dimensional (3D) visualization can see more comprehensively, more clearly and more accurately on the basis of organs and lesions indicated by CT and MRI. Three-dimensional visualization digital medical technology can accurately, intuitively and stereoscopically display the situation of tumor invasion of the adjacent important vessels as well as the variation of vessels. It plays an important role in the aspects of preoperative evaluation, surgical planning, improving the success rate of operation and reducing complications for HCCA. It also transforms the past model of “crossing the river by feeling the stone” to now “crossing the river by watching the stone”. Therefore, it is known as the paradigm of translational medicine.
