摘要
目的 探讨拉曼光谱在诊断肾透明细胞癌中的应用。方法 收集本院2016年6月至2016年10月术后病理确诊为肾肿瘤的38例手术标本,其中肾透明细胞癌标本27例、嗜酸性细胞瘤标本5例及错构瘤标本6例。每例标本随机选取3个不同位点进行光谱检测,利用判别分析法得到拉曼鉴别良恶性肿瘤的准确率。结果 本实验共获得光谱201个,包含:透明细胞癌81个、嗜酸性细胞瘤15个、错构瘤18个、远端肾组织87个。拉曼光谱区分透明细胞癌与嗜酸性细胞瘤及错构瘤的准确率分别为94.20%和87.04%,两者的灵敏度分别为95.06%(77/81)和85.19%(69/81),特异度分别为93.33%(14/15)和 88.89%(16/18)。拉曼光谱区分透明细胞癌与远端肾实质的准确率为93.42%,而良性肿瘤和远端肾实质被拉曼鉴别的准确率均为85.74%。结论 拉曼光谱可用于肾透明细胞癌的鉴别诊断,为肾脏肿瘤的术前诊断提供新方法。
Objective To investigate the availability of Raman spectroscopy for distinguishing renal clear-cell carcinoma (RCC) .Methods There were 27 RCC, 5 oncocytoma and 6 angiomyolipoma were collected in our hospital from June 2016 to October 2016,and then analyzed by Raman spectroscopy. Discriminant Analysis (DA) was applied to evaluate the accuracy of Raman to classify RCC and benign tumors.Results A total of 201 Raman spectra were eventually acquired, of which 81 cases were RCC, 15 cases were oncocytoma, 18 cases were angiomyolipoma, and 87 cases were distal rena parenchyma. Raman was able to recognize RCC and oncocytoma with an accuracy of 94.20%, and distinguish RCC with angiomyolipoma with an accuracy of 87.04%. Besides, the distal renal parenchyma could also be separated from RCC and benign renal tumors with an accuracy of 93.42% and 85.74%, respectively. Conclusions Raman can accurately distinguish RCC and may serve as a useful tool in the future for pre-operative diagnosis of renal tumor.
