色素内镜诊断上消化道早期癌

Application effect of pigment endoscope on diagnosis of early carcinoma of upper digestive tract

目的 探讨色素内镜在上消化道早期癌诊断中的应用效果,为临床诊断提供参考依据.方法 回顾性分析医院行内镜检查的750例患者的一般资料,根据不同病情及检查方式,将行食管黏膜内镜检查的患者360例分为染色1组（行色素内镜检查）与对照1组（行常规内镜检查）,每组180例;将行胃黏膜内镜检查的患者390例分为染色2组（行色素内镜检查）与对照2组（行常规内镜检查）各195例.比较染色1组和对照1组、染色2组和对照2组的病理活检结果 .结果 染色前后早期食管癌,每组的诊断符合率分别为23.81％、80.95％,胃癌的诊断符合率为36.00％、84.00％;染色1组病理活检食管癌诊断率为11.67％,明显高于对照1组的1.11％（P〈0.05）;染色2组病理活检食胃癌诊断率为12.82％,明显高于对照2组的1.03％（P〈0.05）.结论 色素内镜可提高上消化道早期癌的检出率,值得临床重视.

Objective To investigate the application effect of pigment endoscopy on the diagno-sis of upper gastrointestinal cancer,and to provide reference for clinical diagnosis. Methods The gener-al data of 750 patients who underwent endoscopic examination in hospital were retrospectively analyzed;according to disease conditions and inspection methods,360 cases of patients who underwent esophageal mucosal endoscopy were divided into stained 1 group（pigment endoscopy）and control 1 group（conven-tional endoscopy）,with 180 cases in each group;390 cases of patients who underwent endoscopic exami-nation of gastric mucosa were divided into stained 2 group （pigment endoscopy）and control 2 group （con-ventional endoscopy）,with 195 cases ineach group;the pathological biopsy results of stained 1 group and control 1 group,stained 2 group and control 2 group were compared. Results Before and after staining the diagnostic rates of early esophageal cancer were 23. 81%,80. 95% and those were 36. 00%, 84. 00% of gastric cancer,respectively;the diagnosis rate of the pathological biopsy of the stained 1 group was 11. 67%,which was significantly higher than that （1. 11%）in the control 1 group （P 〈0. 05）;the diagnostic rate of pathological biopsy in stained 2 group was 12. 82%,which was significantly higher than that （1. 03%）in the control 2 group（P 〈 0. 05）. Conclusions Pigment endoscopy can im-prove the detection rate of early carcinoma of upper digestive tract,it is worthy of clinical attention.
