经皮侧后路椎间孔镜下髓核摘除术治疗90例腰椎间盘突出症疗效分析

Clinical effects of percutaneous posterior intervertebral foraminoscopy on 90 patients with lumbar intervertebral disc herniation

目的探讨经皮侧后路椎间镜孔下髓核摘除术治疗腰椎间盘突出症的临床效果。方法选取90例腰椎间盘突出症患者为研究对象,按入院顺序将其分为对照组和实验组,每组45例。对照组采取椎板开窗减压髓核摘除术进行治疗,实验组采取经皮侧后路椎间孔镜下髓核摘除术进行治疗,对比分析两组患者的临床疗效。结果实验组术中出血量、手术时间、住院时间、手术切口长度分别为(11.23±3.23)ml、(89.71±5.30)min、(4.50±1.01)d、(0.77±0.32)cm,其手术时间长于对照组,其余相关手术指标均优于对照组,差异有统计学意义(P<0.05);术后随访6个月,两组VAS及ODI评分均较术前下降,但组间比较差异未见统计学意义(P>0.05)。结论腰椎间盘突出症患者采取经皮侧后路椎间孔镜下髓核摘除术进行治疗可缓解临床症状,术后康复快,值得临床推广应用。

Objective To investigate the clinical effects of percutaneous posterior intervertebral foraminoscopy on 90 patients with lumbar intervertebral disc herniation. Methods A total of 90 patients with lumbar intervertebral disc herniation were selected and divided into control group and experimental group according to their hospital admission sequence,with 45 patients in each group. The patients in the control group were treated by the nucleus pulposus resection of lamina fenestration,and the patients in the experimental group were treated by percutaneous posterior intervertebral foraminoscopy. The clinical effects of the two groups were analyzed and compared. Results The intraoperative blood volume,operation time,length of hospitalization and incision length of the patients in experimental group were respectively（ 11. 23 ± 3. 23） ml,（ 89. 71 ± 5. 30） min,（ 4. 50 ± 1. 01） d,（ 0. 77 ± 0. 32） cm. The operation time was longer than that of the control group,and the other related surgical indexes were better than those of the control group,the differences were significant（ P〈0. 05）. After following up for six months,the VAS and ODI scores of both groups were significantly lower than those before operation,but there were no significant differences between two groups,the differences were not significant（ P〈0. 05）. Conclusions In the treatment of patients with lumbar intervertebral disc herniation,percutaneous posterior intervertebral foraminoscopy has significant clinical effects,it can relied fthe clinical symptoms and it＇s worthy of clinical application.
