PDCA循环在全身麻醉开腹手术低体温患者中的应用

The application of PDCA cycle in hypothermia patients with general anesthesia

目的探讨PDCA循环在全身麻醉开腹手术低体温患者中的应用效果。方法选取2015年8月～2016年4月间实施开腹手术的患者80例，按随机数字表法将患者分为实验组和对照组，各40例。实验组采用PDCA循环法为患者提供多途径的保温措施，对照组采用常规手术患者保温措施。比较两组患者入室时、麻醉开始后30min、60min及手术结束时的体温、手术时间、意识恢复时间及拔管时间、术后寒战、躁动及24h伤口引流量等情况。结果两组患者入室时体温及手术时间比较，差异均无统计学意义（均P〉0．05）；实验组患者麻醉开始后30min、60min及手术结束时的体温高于对照组，意识恢复时间及拔管时间短于对照组，术后寒战、躁动及24h伤口引流量低于对照组，差异均有统计学意义（均P〈0．05）。结论PDCA循环法应用于全身麻醉开腹手术患者低体温的预防控制，可有效改善患者体温过低的情况，降低术后并发症的发生率。

Objective To explore the effect of PDCA circulation in patients with hypothermia under general anesthesia. Methods A total of 80 patients who underwent open abdominal surgery between August 2015 and April 2016 were selected, and the patients were divided into experimental group and control group by random number table method, 40 eases of each group. In the experimental group, PDCA cycle method was used to provide the patients with multi-channel insulation measures, and thb control group used the thermal insulation measures of patients with conven- tional operation. The body temperature when entering operation room, 30 minutes, 60 minutes and the end of surgery anesthesia, time of operation, recovery time of consciousness and time of extubation, postoperative cold war, restless- ness and 24 hours wound drainage were compared between the two groups. Results The body temperature when entering operation room and operation time had no statistical difference （P〉0. 05 ） . The temperature of 30 minutes, 60 minutes and the end of surgery anesthesia in experimental group were higher than those of the control group, consciousness recovery time and tube drawing time were shorter than those of the control group, postoperative shivering, agitation and wound drainage 24 hours were lower than those of the control group, and the differences were statistically significant （P〈 0. 05） . Conclusion PDCA circulatory method in hypothermia patients after laparotomy with general anesthesia and control can effectively improve the patient hypothermia situation, and reduce the incidence of postoperative complications.
