摘要
目的应用128层螺旋CT血管成像技术评价冠脉疾病的临床价值。方法回顾性分析我院2016年6月-2017年5月收住的疑有冠脉疾病的患者97例,分析临床及影像资料,对所有患者进行128层螺旋CT冠状动脉成像和选择性冠状动脉造影检查,对结果进行分析。以选择性冠状动脉造影检查作为判断冠脉疾病的金标准,与128层螺旋CT冠脉成像检查结果进行比较。结果 128层螺旋CT冠脉成像血管显示清晰,走形自然;冠状动脉狭窄93例,占95.88%,冠状动脉钙化85例,占87.63%,冠状动脉软斑块68例占70.10%,冠状动脉硬斑块72例,占74.23%;冠脉血管成像诊断冠脉疾病灵敏度为92.39%,特异度为93.75%,阳性预测值为88.54%,阴性预测值为95.93%。并可清晰显示冠脉支架、血管桥及冠脉发育异常。结论 128层螺旋CT冠脉成像无创、安全系数相对较高,该技术对于冠脉血管的显示、狭窄程度判断、支架评价、桥血管显示以及冠脉变异的显示更具有优势,有利于评价冠状动脉疾病。
Objective To evaluate clinical value of 128-slice spiral CT angiography in evaluating coronary artery disease. Methods The clinical and imaging data of 97 cases who were suspectedly underwent coronary artery disease and were admitted to our hospital from June 2016 to May 2017 were analyzed retrospectively. All patients underwent 128-slice spiral CT coronary angiography and selective coronary angiography. The results were analyzed. Selective coronary angiography was used as the gold standard for the diagnosis of coronary artery disease. The results were compared with that of 128-slice spiral CT coronary angiography. Result 128-slice spiral CT coronary artery imaging blood vessels showed clear and natural shape. There were 93 cases with coronary artery stenosis, accounting for 95.88%.There were 85 cases with coronary artery calcification, accounting for 87.63%. There were 68 cases with coronary artery soft plaque, which accounted for 70.10%. There were 72 cases with coronary atherosclerotic plaque, accounting for 74.23%. Sensitivity of coronary artery angiography diagnosis for coronary artery disease was 92.39%, specificity of it was 93.75%. Positive predictive value was 88.54%, and negative predictive value was 95.93%. Coronary stent, vascular bridge and coronary artery anomalies can be clearly displayed. Conclusion128-slice spiral CT coronary angiography is a kind of noninvasive and relatively more safe technology. It has more advantages for coronary angiography, stenosis degree of stent evaluation, bridge blood vessels display and coronary artery variation, and it is a kind of better method to be used to evaluate the coronary artery disease.
