某院2016年辅助用药重点监控药品医嘱点评分析

Analysis of doctor's advice on key monitoring of auxiliary drugs in a hospital in 2016

目的分析某院2016年3-12月辅助用药重点监控品种的医嘱点评情况,为规范管理辅助用药提供参考。方法通过医院信息系统(HIS系统)调取辅助用药重点监控品种的用药数据,分析2016年3-12月辅助用药重点监控品种的销售金额、用药频度(DDDs)、药物利用指数(DUI)等,并对其进行统计分析;对医嘱点评结果进行归纳,总结不合理用药情况,评价点评工作效果。结果 2016年3-12月,不合理用药均为适应证不适宜和用法用量不适宜;药物DUI>1比例下降了10%,在9月和12月实现所有辅助用药重点监控药品未出现在销售排名前10的目标,医嘱点评工作取得一定效果;但仍有30%的药物DUI>1,反映医嘱点评工作仍然存在问题:抽样缺陷、效率低下、与实际脱轨、点评理念不完善。结论某院2016年重点监控辅助用药品种点评取得了一定的效果,形成了制度化、规范化的医嘱点评模式;但仍需加强沟通、多方合作来进一步提升合理用药水平。

Objective To analyze the medication orders of the key monitoring varieties of adjuvant drugs from March to December in 2016 in a hospital,so as to provide reference for the standardized administration of adjuvant drugs.Methods Through the hospital information system（HIS system）,the adjuvant drugs were collected to monitor the drug data of the varieties.The sales amount,DDDs and drug utilization index（DUI） of the key drugs monitored from March to December in 2016 were analyzed statistically.The results of medical advice review were summed up,the irrational drug use situation was summarized,and the evaluation of the work effect was evaluated.Results From March to December in 2016,irrational use of drugs which indicated inappropriate indications and usage;the preportion of drug DUI 〉 1 dropped by 10%;in September and December,all the key monitoring drugs for adjuvant drugs did not appear in the top 10 of the sales ranking,that the work of doctor’s advice had gained a certain effect;but there were still30% of drugs with DUI 〉 1,which reflected that the problems still existed in the work of medical advice review：sampling defect,low efficiency,derailment with practice,and imperfect evaluation idea.Conclusion The hospital has achieved certain results about comments on medication orders on key monitoring varieties of adjuvant drugs in 2016,forming a systematic and standardized mode of doctor’s advice,but its still need strengthen communication and cooperation to further enhance the rational drug use.
