前列地尔对冠心病患者血浆内皮素、同型半胱氨酸和凝血功能的影响

Effect of alprostadil on plasma endothelin,homocysteine and blood coagulation function in patients with coronary heart disease

目的探讨前列地尔对冠心病患者血浆内皮素(ET)、同型半胱氨酸(Hcy)、凝血功能的影响。方法选择东莞市企石医院心内科2014年1月至2017年6月期间收治的150例冠心病不稳定型心绞痛(UAP)患者为研究对象,根据随机数表法分为对照组和观察组,每组75例。对照组予抗血小板等常规治疗,观察组在此基础上联合应用前列地尔治疗,疗程均为14 d。比较两组患者治疗前后的血浆ET、Hcy及D-二聚体、纤维蛋白(FIB)水平的变化。结果治疗后,观察组患者的血浆ET、Hcy水平分别为(80.41±15.48)pg/m L、(13.92±3.15)μmol/L,明显低于对照组的(96.64±17.32)pg/m L、(16.78±3.26)μmol/L,差异均有统计学意义(P<0.05);观察组患者的血浆D-二聚体、Fi B水平分别为(0.36±0.09)mg/L、(2.92±0.64)g/L,明显低于对照组的(0.63±0.12)mg/L、(3.67±0.78)g/L,差异均有统计学意义(P<0.05)。结论前列地尔应用于冠心病的治疗可有效降低患者的血浆ET、Hcy及D-二聚体、FIB水平,进而改善患者的预后。

Objective To investigate the effects of alprostadil on plasma endothelin（ET）,homocysteine（Hcy）and coagulation function in patients with coronary heart disease（CHD）.Methods A total of 150 pateints of coronary heart disease with unstable angina pectoris（UAP） who were treated in Second Department of Internal Medicine,Dongguan Qishi Hospital from January 2014 to June 2017 were selected as the research objects.According to the random number table method,the pateints were divided into observation and control group,with 75 cases in each group.The control group received conventional antiplatelet therapy,and the observation group was treated with alprostadil on the basis of the control group,for 14 days.The changes of plasma levels of ET,Hcy,D-Dimer,and fibrinogen（FIB） between the two groups were compared before and after treatment.Results After treatment,plasma ET,Hcy levels in observation group were respectively（80.41±15.48） pg/m L,（13.92±3.15） μmol/L,which were significantly lower than（96.64±17.32） pg/m L,（16.78±3.26） μmol/L in the control group（P〈0.05）.The plasma D-Dimer and FIB levels in observation group were respectively（0.36±0.09） mg/L,（2.92±0.64） g/L,which were significantly lower than（0.63±0.12） mg/L,（3.67±0.78） g/L in the control group（P〈0.05）.Conclusion Alprostadil can effectively decrease the levels of plasma ET,Hcy,D-Dimer,and FIB in patients with coronary heart disease,and thus improve the prognosis.
