110例非糜烂性胃食管反流病患者中医证候特征分析

Analysis on the Traditional Chinese Medicine Syndrome Characteristics of Non Erosive Gastroesophageal Reflux Disease for 110 Cases

目的探讨非糜烂性胃食管反流病的中医证候特征,为非糜烂性胃食管反流病的中医治疗提供辨证施治的依据。方法采用前瞻性流行病学的调查方法,对110例非糜烂性胃食管反流病患者的中医症状、体征、舌、脉等四诊信息,对患者进行中医辨证,统计分析这类患者的证候学特点。结果非糜烂性胃食管反流病患者常见的中医证候依次为:肝胃郁热证(45.45%)、痰气交阻证(34.55%)、肝郁气滞证(20%)、中虚气逆证(19.09%)、肝胃不和证(18.18%)、脾肾阳虚证(9.09%)、胆热犯胃证(9.09%)、肝郁脾虚证(8.18%)、瘀血阻络证(7.27%)、痰热内蕴证(7.27%)、脾胃气虚证(6.36%)、脾胃湿热证(5.45%)、胃阴亏虚证(3.64%)、脾胃阳虚证(2.73%)。其中同一个患者常有几个证候重叠。结论非糜烂性胃食管反流病患者的中医证候较少见单一证候,兼夹证多见。肝胃郁热兼痰气交阻的复合证型,是非糜烂性胃食管反流病的最特征性中医证候。

Objective To investigate the characteristics of TCM Syndromes of non-erosive gastroesophageal reflux disease,and to provide evidence for the treatment of non-erosive gastroesophageal reflux disease. Methods By means of prospective epidemiological survey,the four diagnosis of TCM symptom,physical sign,tongue and pulse from 110 patients with non erosive gastroesophageal reflux disease were extracted and differentiations of symptoms and signs from patients were taken. Results TCM syndromes type of non-erosive gastroesophageal reflux disease are as follows： heat stagnation of liver and stomach（45. 45%）,gas exchange resistance syndrome（34. 55%）,stagnation of liver qi（20%）,syndrome of deficiency of qi in the interior qi（19. 09%）,liver stomach disharmony syndrome（18. 18%）,spleen and kidney yang deficiency（9. 09%）,gallbladder heat invading stomach syndrome（9. 09%）,stagnation of liver qi and spleen deficiency（8. 18%）,blood stasis obstructing collaterals（7. 27%）,phlegm heat accumulation syndrome（7. 27%）,qi deficiency of the spleen and stomach（6. 36%）,spleen stomach damp heat syndrome（5. 45%）,deficiency of stomach yin（3. 64%）,syndrome of yang deficiency of the stomach and spleen（2. 73%）. Among them,one patient often had several overlap syndromes. Conclusion The TCM syndrome type of non-erosive gastroesophageal reflux disease is rare single type,but more overlap. The characteristic of the TCM syndrome type is liver stomach heat and gas exchange resistance.
