围术期系统化护理在脑肿瘤患者中应用 被引量:3

摘要 目的了解围术期系统化护理脑肿瘤患者中的应用效果。方法选取145例择期脑肿瘤手术患者,采用随机数字表法分为观察组73例和对照组72例。对照组给予围术期常规护理措施,观察组在此基础上给予系统化护理。比较两组患者入院时与出院时负性情绪、住院期间并发症(感染、颅内血肿、脑水肿)、入院时与出院1个月时日常活动能力和生活质量差异。结果出院时,观察组和对照组汉密尔顿抑郁量表(HAMD)评分(15.32±2.54)分、(17.64±2.73)分与汉密尔顿焦虑量表(HAMA)评分(8.32±1.65)分、(10.35±1.96)分均较入院时降低,且观察组低于对照组,差异均有统计学意义(P<0.05)。出院1个月时,观察组和对照组Karnofsky体能状况评分量表(KPS)评分(76.45±3.15)分、(72.69±3.29)分与脑肿瘤生命质量测评量表(QLICP-BN)评分(63.15±3.34)分、(61.26±3.61)分均较入院时升高,且观察组高于对照组,差异均有统计学意义(P<0.05)。住院期间,观察组相关并发症发生率(感染5.48%、颅内血肿1.37%、脑水肿2.74%)均较对照组(16.67%、9.72%、11.11%)低,差异有统计学意义(P<0.05)。结论围术期系统化管理可缓解脑肿瘤手术患者负性情绪,降低术后并发症,提高患者生活质量。 Objective To investigate the application effect of the perioperative systematic nursing management in patients with brain tumor. Methods 145 cases of brain tumor in our hospital were selected as study objects and were divided into systematic nursing group( observation group,n = 73) and routine nursing group( control group,n = 72). The control group was given preoperative preparation,postoperative guidance,and the observation group was given systematic nursing management on basis of the control group. Negative emotion( HAMD scale),HAMA scale,complications during hospitalization( infection,intracranial hematoma,encephaledema) of the two groups at admission and discharge were compared. Daily living activity( KPS scale),life quality( QLICP-BN) of the two groups at admission and 1 month after discharge were recorded. Results When discharged,HAMD( 15. 32 ± 2. 54) 、( 17. 64 ±2. 73) and HAMA( 8. 32 ± 1. 65) 、( 10. 35 ± 1. 96) scores of the two groups were all declined,and the observation group was lower than that of the control group. The difference was statistically significant( P 0. 05). One month after discharge,KPS( 76. 45 ± 3. 15) 、( 72. 69 ± 3. 29) and QLICP-BN( 63. 15 ±3. 34) 、( 61. 26 ± 3. 61) scores of the two groups were all increased compared with those at admission( P 0. 05),and the observation group was higher than that of the control group( P 0. 05). The difference was statistically significant. During hospitalization,complication occurrence rate of the observation group was lower than that of the control group. The difference was statistically significant( P 0. 05).Conclusion Perioperative systematic nursing management can relieve negative emotion of patients withbrain tumor,lower postoperative complications and is benefit for improving life quality.
