IL-6和hs-CRP在2型糖尿病合并不同类型冠心病患者中的检测意义

Detection significance of IL-6 and hs-CRP in type 2 diabetes mellitus patients with different types of coronary heart disease

目的分析2型糖尿病合并不同类型冠心病患者中血清炎症因子IL-6和超敏C反应蛋白(hs-CRP)的表达水平,为临床诊断和治疗提供参考依据。方法选择2013年5月~2014年5月入住我院确诊为2型糖尿病合并冠心病患者168例,其中稳定型心绞痛(SAP)组46例,不稳定型心绞痛(UAP)组52例和急性心肌梗死(AMI)组70例;测定血清IL-6、hs-CRP水平,并行冠状动脉造影和血管内超声检查。结果 AMI组IL-6和hs-CRP水平最高,其次UAP组,SAP组最少,组间比较差异有统计学意义(P<0.05)。血管直径狭窄程度与IL-6和hs-CRP水平呈正相关(P<0.05)。脂质性斑块IL-6和hs-CRP水平显著高于混合性斑块和硬斑块(P<0.05)。结论 IL-6和hs-CRP的表达水平随着冠心病患者管腔的狭窄加重,斑块不稳定而逐渐增加,对2型糖尿病合并不同类型冠心病的诊断和治疗有一定的参考价值。

Objective To investigate the expression of serum inflammatory cytokines IL-6 and hs-CRP in type 2 diabetes mellitus patients with different types of coronary heart disease（CHD） and provide a reference for clinical diagnosis and treatment. Methods A total of 168 patients diagnosed with type 2 diabetes mellitus complicated with coronary heart disease admitted in our hospital from May 2013 to May 2014 were continuously selected, including stable angina（SAP） group（n =46）, unstable angina（UAP） group（n =52） and acute myocardial infarction（AMI） group（n =70）.Serum levels of IL-6 and hs-CRP were measured. Coronary angiography and intravascular ultrasound were performed.Results The levels of IL-6 and hs-CRP were the highest in AMI group, followed by those in the UAP group, with the lowest in SAP group, and the difference was statistically significant among the groups（P〈0.05）. The degree of vessel diameter stenosisbe was positively correlated with levels of IL-6 and hs-CRP（P〈0.05）. The levels of IL-6 and hs-CRP in the lipid plaques were significantly higher than those in the mixed plaques and hard plaques（P〈0.05）. Conclusion The expression levels of IL-6 and hs-CRP in patients with type 2 diabetes mellitus（T2 DM） gradually increase with the increase of lumen stenosis and plaque instability, which has somereference value in the diagnosis and treatment of type2 diabetes mellituscomplicated with different types of coronary heart disease.
