产妇安合剂联合自由体位干预对初产妇第二产程及分娩结局的影响

Effect of Chanfu'an Mixture Combined with Free Position on the Length of the Second Stage of Labor and Delivery Outcome of Primiparas

目的探讨产妇安合剂口服液联合自由体位干预对初产妇第二产程时长及分娩结局的影响。方法选择医院2016年5月至2017年7月收治的欲分娩初产妇80例,采用随机数字表法分为两组,每组40例。两组患者均由同一组医护人员帮助分娩,两组产妇均采用自由体位,观察组产妇在分娩期前4 d开始口服产妇安合剂,每日3次,每次25 m L,直至分娩结束当日,共用药5 d。结果观察组产妇术第二产程、产妇自我用力分娩时间均短于对照组,分娩过程中出血量少于对照组(P<0.05);两组产妇会阴和分娩情况比较,差异无统计学意义(P>0.05);两组产妇主要分娩及产后并发症为产后出血、产后发热、伤口愈合不良、胎膜早破,观察组分娩及产后并发症发生率低于对照组(55.00%比80.00%,P<0.05);两组新生儿主要并发症为巨大儿、新生儿窒息、新生儿住院、新生儿酸血症,两组新生儿并发症发生率相当(57.50%比62.50%,P>0.05);观察组不良反应发生率虽高于对照组,但差异无统计学意义(P>0.05)。结论产妇安合剂口服液联合自由体位干预可缩短初产妇第二产程时长,对产妇分娩结局有积极影响,可有效减少产妇分娩及产后并发症、新生儿并发症发生率,且安全性高,值得临床推广。

Objective To investigate the effect of Chanfu＇an Mixture combined with free position on the length of the second stage of labor and deliveFy outcome of primiparas. Methods Totally 80 primiparas admitted to our hospital from May 2016 to July 2017 were selected and divided into two groups by random number table method,40 cases in each group. The primiparas in the two groups were assisted by the same group of medical staffs,and the primiparas in the two groups were taken free positlons. The primiparas in the observation group began to take Chanfu＇an Mixture from 4 d before deliveFy to the end of delivery,3 times/day,25 mL/time,a total of 5 d. Results The time of self-exertion deliveFy in the second stage of labor in the observation group was shorter than that in the control group,and the amount of bleeding during deliveFy in the observation group was less than that in the control group（P 〈 0.05）. There was no significant difference in perineum and deliveFy between the two groups（P 〉 0.05）. The main complications of deliveFy and postpartum in the two groups were postpartum hemorrhage, postpartum fever, poor wound healing and premature rupture of membranes ,theincidence rate of total complications of delivery and postpartum in the observation group was significantly lower than that in the control group（55.00% vs. 80. 00%, P 〈 0.05）. The main neonatal complications in the two groups were macrosomia,neonatal asphyxia,neon- atal hospitalization, neonatal acidemia, there was no significant difference in the incidence of neonatal complications between the two groups（57.50% vs. 62. 50%, P 〉 0. 05）. The incidence rate of adverse reactions in the observation group was higher than that in the control group,but the difference was not statistically significant（P 〉 0.05）. Conclusion Chanfu＇an Mixture combined with fi＇ee position can shorten the length of the second stage of labor for primiparas,it has a positive effect on the delivery outcomes of primiparas,it can effectively reduee the delivery and postpartum complications and neonatal complications. It has high safety and is worthy of clinical promotion.
