急性脑梗死患者脑小血管病总体负担与颅内大血管病变的关系及影响因素研究

A Study of the Relationship between the Total Burden of Cerebral Small Vessel Disease and Intracranial Macroangiopathy in Patients with Acute Cerebral Infarction and Its Influencing Factors

目的探讨急性脑梗死患者脑小血管病总体负担与颅内大血管病变的关系,并研究其影响因素;方法选取2016年07月至2018年01月期间就诊,并完成高分辩率核磁共振成像(HR-MRI)检查的急性脑梗死患者,采用"总SVD评分法"和"NASCET法"评价患者脑小血管病总体负担和颅内血管狭窄程度,选用非参数检验、卡方检验和Spearman秩相关统计分析;结果 (1)脑小血管病总体负担评分各组间年龄、高血压患病率差异有统计学意义(P<0.05),但各分组间糖尿病、高脂血症及心脏病患病率差异无统计学意义(P>0.05);(2)脑小血管病总体负担评分与颅内大血管狭窄程度呈正相关性(r=0.474,P<0.05),有统计学意义。结论脑小血管病总体负担与颅内大血管狭窄程度呈正相关,年龄、高血压为脑小血管病总体负担的影响因素。

Objective The objective of this study is to explore the relationship between the total burden of cerebral small vessel disease and intracranial macroangiopathy in patients with acute cerebral infarction,and the influencing factors that contribute to cerebral small vessel disease.Methods The data of this study came from the patients with acute cerebral infarction who had received treatment especially HR-MRI examination from July2016 to January 2018.The ＂total SVD score＂and ＂NASCET method＂were used to evaluate the overall burden of cerebral vasculopathy and intracranial vascular stenosis.To analyze the collected data,non-parametric test,chi-square test and Spearman rank correlation analysis were performed. Results（1） There was a significant difference in the age and the prevalence of hypertension among the groups in the total burden of cerebral vascular disease（ P〈0. 05）. However,there was no statistical difference in the prevalence of diabetes,hyperlipidemia and heart disease among the subgroups（ P〉0. 05）;（2） The total burden score of cerebral small vessel disease was positively correlated with the severity of intracranial vascular stenosis with statistical significance（ r = 0. 474,P0. 05）.Conclusion Our study demonstrated a positive correlation between the overall burden of cerebral small vessel disease and the degree of stenosis of large vessels in the brain. Furthermore,we also found that age and hypertension are the influencing factors of the overall burden of cerebral small vessel disease.
