体质指数与2型糖尿病高危人群心血管功能的相关性分析

Correlation between body mass index and cardiovascular function in people with high risk of type 2 diabetes mellitus

目的探讨体质指数(BMI)与2型糖尿病(T2DM)高危人群的心脏及血管功能的变化及其相关性,为早期预防提供依据。方法选取2016年8月至2017年1月新疆医科大学健康管理院个案管理部304位接受无创主动脉脉波分析仪检测的人员,将其按BMI分为正常组和高危组。运用t检验,方差分析,Logistic回归分析及偏相关分析。结果两组间心功能各项指标的比较:中心动脉压在两组间差异有统计学意义(P<0.05),Logistic回归分析,SBP、DBP及腰围为心功能是否合格的危险因素(OR>1)。糖尿病高危人群的各项高危指标与心功能的相关性分析,年龄、BMI、血压、腰围臀围这些T2DM的高危因素与心功能有相关性,年龄与动脉硬度(CSBP、CPP、AP、AIx)及ED呈正相关(P<0.05);BMI与SBP、DBP及CSBP、CPP、AP呈正相关(P<0.05);腰围与SBP、DBP及动脉硬度呈正相关(P<0.05)。结论 BMI与肱动脉血压(SBP、DBP)及CSBP、CPP、AP呈正相关。腰围与动脉硬度及肱动脉血压呈正相关。向心性肥胖是中心血压及动脉硬度指标的重要决定因素。

Objective To investigate the changes of heart mass and vascular function in body mass index（ BMI） and type 2 diabetes mellitus（ T2 DM）,and to provide evidence for early prevention. Methods From August 2016 to January 2017,304 patients were randomly divided into normal group and high risk group according to BMI. The patients were treated with noninvasive aortic pulse wave analyzer. The t test,variance analysis,logistic regression analysis and partial correlation analysis were used. Results The central arterial pressure between the two groups was statistically significant（ P 0. 05）. Logistic regression analysis showed that SBP,DBP and waist circumference were risk factors for cardiac function（ OR 1）.（ Age,BMI,blood pressure,waist circumference） The risk factors of these T2 DM were correlated with cardiac function,age and arterial stiffness（ CSBP,CPP,AP,AIx） in patients with high risk of diabetes mellitus（ P 0. 05）. There was a positive correlation between BMI and SBP,DBP and CSBP,CPP and AP（ P 0. 05）. There was a positive correlation between BMI and SBP,DBP and arterial stiffness（ P 0. 05）. Conclusion BMI is positively correlated with brachial artery blood pressure（ SBP,DBP） and CSBP,CPP and AP. Waist circumference and arterial stiffness and brachial artery blood pressure was positively correlated. Concentric obesity is an important determinant of central blood pressure and arterial stiffness.
