肿瘤专科临床药师对一例胃癌患者的药学监护与实践

Pharmaceutical care and practice for a patient with gastric cancer by clinical pharmacists in oncology department

目的探讨肿瘤专科临床药师如何为患者实施药学监护。方法以1例65岁男性胃癌复发患者为例,临床药师在治疗过程中主动参与临床,协助医生制定化疗方案,综合分析本次及既往化疗方案,结合辅助用药情况,查找不良反应原因,为临床提出合理性解决建议,对患者治疗进行跟踪监测并提供用药教育。结果患者化疗过程顺利完成,用药安全得到有效保障。结论在新医改背景下,临床药师可以在治疗团队中发挥更大的作用,使药物治疗更趋合理,提高临床药物治疗水平。

Objective To explore how, to carry out pharmaceutical care for patients by clinical pharmacists in oncology department. Methods Clinical pharmacists participated in the clinical treatment for a patient with recurrent gastric cancer, assisted clinicians in making chemotherapy regimen, analyzed current and previous chemotherapy and adjuvant drugs, assessed the causes of adverse drug reactions, gave reasonable suggestions for clinicians, and provided fol- low-up and medication education for the patient. Results The chemotherapy of the patients completed successfully, and the drug safety was ensured. Conclusion Clinical pharmacist can play a greater role in treatment team,improving the rationality and therapeutic level of medication.
