某市属三级综合医院37785例住院患者费用分析

Analysis of the Cost of 37785 Hospitalized Patients in Three Level General Hospital

目的了解某院住院患者费用的影响因素,为降低医院住院患者费用提供参考建议。方法选用描述性分析、秩和检验、多重线性回归分析对2017年1月1日-2017年12月31日37 785例住院患者的基本信息、费用及影响因素进行统计分析。结果住院天数对住院费用的影响最大,住院天数越长,住院费用越高(标准化回归系数=0.5,P<0.001);社会特征中的年龄越大住院费用越高(P<0.001);病危重患者(25207.3±32 450.5)住院费用高于非危重患者(11 517.5±12 322.7)(P<0.001),疑难病患者(34 044.6±42 484.1)住院费用高于非疑难病患者(14 242.6±18 896.7)(P<0.001)。结论重视老年患者的健康问题、缩短平均住院日、重视影响费用增长的其他因素。

Objective To understand the factors affecting the cost of hospitalized patients in a hospital and to provide suggestions for reducing the cost of hospital inpatients. Methods Descriptive analysis, rank sum test and multiple linear regression analysis were used to analyze the basic information, expenses and influencing factors of37 785 inpatients from January 1, 2017 to December 31 st. Results The number of hospitalization days has the greatest impact on the cost of hospitalization, the longer the days of hospitalization, the higher the cost of hospitalization（the normalized regression coefficient =0.5, P〈0.001）. The higher the age of the social characteristics, the higher the cost of hospitalization（P〈0.001）. The hospitalization expenses of critically ill patients（25 207.3＋32 450.5） were higher than those of non critical patients（11 517.5＋12 322.7）（P〈0.001）. The cost of hospitalized patients（34 044.6＋42 484.1） was higher than that of non-difficult patients（14 242.6＋18896.7）（P〈0.001）. Conclusions Pay attention to the health problems of elderly patients, shorten the average days of hospitalization, and attach importance to other factors that affect the growth of the cost.
