领悟社会支持对初产妇产后抑郁症的影响研究

The impact of perceived social support on the early postpartum depression of primiparas

目的调查初产妇产后抑郁现状,并探讨领悟社会支持对产后抑郁的影响。方法 2016年7~8月,采用横断面研究,在杭州某妇产科医院住院病房对符合入选标准的初产妇611例进行问卷调查。应用爱丁堡产后抑郁量表对分娩初期抑郁进行评估,应用领悟社会支持量表评估领悟社会支持,应用logistic回归分析校正混杂因素后的领悟社会支持和产后抑郁的关系。结果初产妇产后抑郁(得分≥9分)发生率为17.35%(106/611),领悟社会支持总分(69.12±9.14)分;领悟社会支持总分高分组产后抑郁发生率(χ~2=13.96,P<0.05)和得分(t=6.46,P<0.05)均低于低分组,差异有统计学意义;在完全校正模型里,高领悟社会支持可降低初产妇的产后抑郁风险(OR=0.56,P=0.011)。结论领悟社会支持对分娩初期抑郁的影响较大,其中朋友支持得分与产后抑郁得分关系最密切。

Objective To discuss the impact of perceived social support（PSS）on the depression of primiparas during the early postpartum period. Methods A cross-sectional study was designed to conduct a questionnaire survey among 611 primiparas who met the inclusion criteria and were hospitalized in a Maternal Hospital in Hangzhou.Postpartum depression（PD）was assessed using the Edinburgh Postnatal Depression Scale（EPDS）and PSS was assessed by using the Perceived Social Support Scale（PSSS）.Logistic regression was computed to assess the relationship between PSS and PD after adjusting for confounding variables. Results The prevalence of PD（score≥9）was 17.35%（106/611）,and the total score of PSS was 69.12±9.14 among Chinese primiparas.Postnatal women with high PSS score had lower prevalence of PD,and lower PD score than those with low PSS score,the differences were statistically significant（χ~2=13.96,t=6.46,P〈0.05）.In the fully adjusted model,primiparas with higher PSS usually had lower risk of PD（OR=0.56,P=0.011）.Conclusions PSS has significant impact on the early postpartum depression of piriparas,support of friend is closely related to PD score.
