结合雌激素乳膏对老年性阴道炎患者临床症状及性激素水平的影响

Comparative study on triptorelin acetate and mifepristone in postoperative treatment of endometriosis

目的探讨结合雌激素软膏对老年性阴道炎患者临床症状及性激素水平的影响,为临床用药提供指导。方法选取2015年9月—2016年12月收治的92例老年性阴道炎患者作为研究对象,利用随机数表法分为对照组和观察组,各46例。对照组给予甲硝唑栓治疗,观察组在使用甲硝唑栓的基础上给予结合雌激素乳膏治疗,比较两组治疗效果、治疗前后临床症状改善情况以及性激素水平的变化情况,并记录所有患者治疗期间不良反应发生率及复发率。计量资料比较采用t检验,计数资料比较采用χ~2检验,P<0.05为差异有统计学意义。结果对照组治疗有效率为71.74%,低于观察组的95.65%,复发率为30.43%,高于观察组的6.52%,差异有统计学意义(均P<0.05)。与治疗前相比,两组阴道炎症评分、阴道健康状况评分、阴道脱落细胞成熟指数均明显改善,且观察组治疗后阴道炎症评分低于对照组,阴道健康状况评分、阴道脱落细胞成熟指数高于对照组,差异有统计学意义(均P<0.05);与治疗前比较,两组性激素水平均明显改善,且观察组治疗后FSH水平高于对照组、E2水平低于对照组,差异有统计学意义(均P<0.05);两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论结合雌激素乳膏在治疗老年性阴道炎中可有效提高治疗效果,缓解临床症状,还可改善性激素水平,减少复发率。

Objective To investigate the effect of conjugated estrogens cream on clinical symptoms and sex hormone level in patients with senile vaginitis, providing guidance for clinical medication.Methods A total of 92 patients with senile vaginitis collected from September 2015 to December 2016 were selected, and they were divided into two groups by random number table, 46 cases in each group. Control group was given metronidazole suppositories, while observation group was treated with conjugated estrogens cream based on control group. The curative effects, improvement of clinical symptoms and changes of sex hormone level were compared between the two groups. The adverse reactions and recurrence during treatment were recorded. Measurement data were compared by t test and enumeration data were compared by χ-2 test.P〈0.05 was considered statistically significant.Results The effective rate of control group was 71.74%, which was lower than 95.65% of observation group, and the recurrence rate of control group was 30.43%, which was higher than 6.52% of observation group（all P〈0.05）. Compared with before treatment, the scores of vaginal inflammation, vaginal health status, maturation index of vaginal exfoliated cell in two groups improved, and after treatment, the score of vaginal inflammation in observation group was lower than that in control group, while the score of vaginal health status and maturation index of vaginal exfoliated cell in observation group were higher than those in control group（all P〈0.05）.Compared with before treatment, the sex hormone levels of two groups improved, and after treatment, the FSH level of observation group was higher than that of control group, while the E2 level of observation group was lower than that of control group（all P〈0.05）. There was no statistical difference in the adverse reactions between the two groups（P〈0.05）.Conclusion Conjugated estrogens cream in treatment of senile vaginitis can increase curative effects, relieve clinical symptoms, improve sex hormone level and reduce recurrence.
