依达拉奉联合尿激酶静脉溶栓治疗急性脑梗死效果观察

Clinical efficacy of edaravone combined with urokinase thrombolysis in treatment of acute cerebral infarction

目的探究依达拉奉联合尿激酶静脉溶栓治疗急性脑梗死的临床疗效,为临床选择合适的治疗方案提供依据。方法选择2016年3月—2017年2月收治的急性脑梗死患者112例,根据随机数字表法分为两组,各56例。对照组给予尿激酶静脉溶栓治疗,观察组采用依达拉奉联合尿激酶静脉溶栓治疗,2个疗程后比较两组血清炎性因子水平、临床疗效、氧化应激指标、日常生活能力及神经功能缺损评分、药物不良反应。计量资料比较采用t检验,计数资料比较采用χ2检验,P<0.05为差异有统计学意义。结果与对照组相比,观察组治疗总有效率较高,差异有统计学意义(P<0.05);治疗后,与对照组相比,观察组hs-CRP、TNF-α、MDA、NSE水平、NIHSS评分均较低,ADL评分、SOD水平均较高,差异具有统计学意义(均P<0.05);两组治疗期间均未出现严重的药物不良反应。结论急性脑梗死患者采用依达拉奉联合尿激酶静脉溶栓治疗可提高临床疗效,减轻炎症反应及氧化应激反应,改善患者日常生活活动能力及神经功能。

Objective To investigate the clinical efficacy of edaravone combined with urokinase thrombolysis in treatment of acute cerebral infarction（ACI）, providing the basis for the clinical selection of the appropriate treatment.Methods A total of 112 ACI patients collected from March 2016 to February 2017 were divided into two groups by random number table, 56 cases in each group. Control group was given intravenous thrombolysis with urokinase, while observation group was treated with edaravone based on control group. After two courses, the inflammatory factors, clinical efficacy, oxidative stress indexes, activity of daily living, neurological deficit score, adverse reactions were compared between the two groups. Measurement data were compared by t test and enumeration data were compared by χ2 test.P〈0.05 was considered statistically significant.ResultsCompared with control group, the total effective rate of observation group was higher（P〈0.05）. After treatment, compared with control group, the hs-CRP, TNF-α, MDA, NSE levels and NIHSS scores in observation group were lower, while ADL scores and SOD level in observation group were higher（P〈0.05）. There were no severe adverse reactions in two groups during treatment.Conclusion Edaravone combined with urokinase thrombolysis in treatment of ACI can increase clinical efficacy, reduce inflammatory reaction and oxidative stress response, improve activity of daily living and nerve function.
