中药保留灌肠对急性放射性肠炎的疗效及护理研究

Clinical Efficacy and Nursing Experience of Retention Enema with Chinese Herbal Medicine on Acute Radiation Enteritis

目的观察中药保留灌肠对急性放射性肠炎的临床疗效并总结护理体会。方法 70例患者按随机数字表法分为对照组与治疗组各35例。对照组给予谷氨酰胺颗粒口服治疗,治疗组在对照组的基础上给予中药保留灌肠的护理方法,分别观察两组治疗前后卡氏功能评分(KPS)、临床症状积分及免疫因子(CD3^+、CD4^+、CD8^+T淋巴细胞亚群)的变化,并评价两组患者的临床疗效。结果治疗组总有效率为85.7%,高于优于对照组的77.1%(P<0.05)。两组治疗后KPS评分与本组治疗前比较均增高(均P<0.01),且治疗后治疗组优于对照组(P<0.01)。两组治疗后临床症状积分与本组治疗前比较均降低(P<0.01),且治疗后治疗组改善优于对照组(P<0.01)。两组治疗后CD3^+、CD4^+T淋巴细胞的比例及CD4^+/CD8^+的比值与本组治疗前比较均增高,CD8^+T淋巴细胞的比例降低(均P<0.01),且治疗组治疗后CD3^+淋巴细胞、CD4^+、CD8^+及CD4^+/CD8^+的比值均改善优于对照组(P<0.01或P<0.05)。结论中药保留灌肠能够明显改善急性放射性肠炎的临床症状,改善肿瘤患者的生存质量和免疫功能,疗效显著。

Objective： To observe the clinical efficacy and nursing experience of retention enema with Chinese herbal medicine on acute radiation enteritis. Methods： 70 cases with acute radiation enteritis were randomly divided into the control group（35 cases） and the treatment group（35 cases）. The control group was treated with Glutamine Granules and the treatment group was nursed with retention enema of TCM on the basis of the control group. The changes of KPS score,clinical symptom score and immune factors（CD3＋,CD4＋,CD8＋T lymphocyte subsets） were observed before and after treatment in the two groups,and the clinical efficacy of the two groups was evaluated after treatment. Results： The total effective rate of the treatment group was 85.7%,which was significantly higher than that of the control group（P 〈 0.05）;KPS score was increased significantly（P 〈 0.01） and the clinical symptom score was decreased significantly in the treatment group after treatment（P0.01）,which was significantly better than that of the control group（P 〈 0.01）. The content of CD3＋,CD4＋T lymphocytes and the ratio CD4＋/CD8＋were increased significantly while the content of CD8＋ was decreased significantly after treatment in the treatment group（P 〈 0.01）. The content of CD3＋in the treatment group was significantly better than that in the control group（P 〈 0.01） and the content of CD4＋,CD8＋and the ratio CD4＋/CD8＋were better than that in the control group（P 〈 0.05）. Conclusion： Retention Enema with Chinese medicine can significantly improve the clinical symptoms of acute radiation enteritis,the quality of life and immune function of patients with tumor,and the curative effect is remarkable.
