凉血解毒退银汤对急性银屑病患者免疫功能、炎症因子及生活质量的影响

Effects of Liangxue Jiedu Tuiyin Decoction on Immune Function,Inflammatory Factors and Quality of Life in Patients with Acute Psoriasis

目的观察自拟凉血解毒退银汤对急性银屑病患者免疫功能、炎症因子及生活质量的影响。方法将急性银屑病(血热证)患者86例按随机数字表法分为对照组与观察组各43例。对照组给予他扎罗汀凝胶和强的松治疗,观察组在对照组基础上给予自拟凉血解毒退银汤口服治疗。4周后观察两组患者的临床疗效、中医证候积分、免疫功能、炎症因子及生活质量。结果治疗后,观察组总有效率为95.35%,高于对照组的79.07%(P<0.05)。观察组的瘙痒、红斑、皮损、心烦、咽喉肿痛等中医证候积分均显著低于对照组(P<0.05)。观察组的外周血CD3、CD4和CD4/CD8均明显高于对照组(P<0.05)。观察组的血清白细胞介素-6、白细胞介素-23和肿瘤坏死因子-α水平显著低于对照组(P<0.05)。观察组患者的躯体、认知、情绪功能、角色和社会功能各项评分均显著高于对照组(P<0.05)。结论自拟凉血解毒退银汤对急性银屑病患者具有较好的临床疗效,可改善患者的中医证候积分,增强免疫功能,减轻炎症反应,提高生活质量。

Objective： To explore the effects of Liangxue Jiedu Tuiyin Decoction on immune function,inflammatory factors and quality of life in patients with acute psoriasis. Methods： 86 cases of patients with acute psoriasis of blood heat type were randomly divided into the control group（43 cases） and the observation group（43 cases）. The control group was given Tazarotene Gel and prednisone in the treatment,and the observation group were added Liangxue Jiedu Tuiyin Decoction oral treatment. After 4 weeks,the clinical efficacy,TCM syndrome scores,immune function,inflammatory factors and quality of life were observed in the two groups. Results： After treatment,the total effective rate of the observation group was 95.35%,and the total effective rate of the control group was 79.07%（P 〈 0.05）. The TCM syndrome scores of pruritus,erythema,skin lesions,irritability and sore throat in the observation group were significantly lower than those in the control group,and the difference was statistically significant（P 〈 0.05）. The CD3,CD4 and CD4/CD8 in the peripheral blood of the observation group were significantly higher than those in the control group,and the difference was statistically significant（P 〈 0.05）. The levels of serum interleukin-6,interleukin-23 and tumor necrosis factor-α in the serum of the observation group were significantly lower than those in the control group,and the difference was statistically significant（P 〈 0.05）. The scores of the body,cognition,emotional energy,role and social function of the observation group were significantly higher than those in the control group,and the difference was statistically significant（P 〈 0.05）. Conclusion：Liangxue Jiedu Tuiyin Decoction has a good clinical effect on patients with acute psoriasis,which can improve TCM syndrome score,enhance immune function,reduce inflammatory reaction and improve quality of life.
