联合部分筛骨垂直板在鼻美容整形术中的应用

The application of joint partethmoid bone vertical platein cosmetic rhinoplasty

目的探讨筛骨垂直板应用于鼻美容整形术中的方法、效果及其优缺点。方法对35例鼻部有美学缺陷的求美者,施行常规开放入路鼻美容整形术。术中取部分筛骨垂直板加鼻中隔软骨和耳软骨重建强有力的鼻尖支架,再结合鼻背假体隆鼻。术后随访观察是否出现相关并发症,并对术后鼻部美学效果进行评价。结果术后随访3~9个月,所有求美者均未见其他并发症发生。鼻美学效果评价:32例满意;3例欠满意,其中2例对鼻背高度欠满意,1例出现轻微鼻背假体歪斜,总体对鼻尖形态满意。结论联合应用部分筛骨垂直板在一定程度上解决了鼻美容整形术中材料不足的问题,操作安全可行,美学效果较佳,求美者的满意度较高。

Objective To explore the methods, efficacy and relative merits of the application of joint part ethmoid bone vertieal plate in cosmetic rhinoplasty. Methods Thirty-five patients with nasal cosmetic defects engaged in this study underwent an external approach rhinoplasty. A fraction of the perpendicular plate of the ethmoid was eul off for the purpose of a tip strut reconstruction combined with septal and conehal carlilage, and the nasal dorsum was shaped by a prosthesis. Follow-up lasted three to nine monlhs to observe if complications occurred. The cosmetic effects were also evaluated postoperatively. Results After a follow-up of three to nine months, an evident loss of nasal tip projection but no other complications were observed in two of the thirty-five patients. Thirty-two palients expressed their satisfaction with the surgery but three did not. Two complained of an insufficient height of nasal dorsum, and one had prosthesis deflection in the nasal dorsum. All patients were satisfied with the shape of the nasal tip. Conclusion Combining part of the perpendicular plate of the ethmoid bone can provide more available material in cosmetic rhinoplasty. It can he applied in a safe and effective way, with a suitable cosmetic effect and high degree of patient satisfaetion.
