正本清源汤联合米非司酮治疗围绝经期功能性子宫出血45例

Zheng Ben Qing Yuan Decoction Combined with Mifepristone in Treating 45 Patients with Dysfunctional Uterine Bleeding during Peri-menopausal Period

目的:观察正本清源汤联合米非司酮治疗围绝经期功能性子宫出血的临床疗效及其对子宫内膜厚度的影响。方法:将90例围绝经期功能性子宫出血患者随机分为观察组和对照组,每组45例。观察组于刮宫术后第5天给予正本清源汤联合米非司酮治疗,对照组于刮宫术后第5天给予米非司酮治疗,2组均治疗3个月经周期,比较2组控制出血时间、完全止血时间、卵泡刺激素(FSH)、黄体生成素(LH)、雌二醇(E_2)以及子宫内膜厚度。结果:控制出血时间、完全止血时间观察组短于对照组,2组比较差异有统计学意义(P<0.05)。FSH、LH以及E_2水平治疗后均显著低于治疗前,治疗前后2组组内比较差异有统计学意义;治疗后E_2水平2组组间比较差异有统计学意义(P<0.05)。子宫内膜厚度治疗后2组均显著小于治疗前,差异有统计学意义(P<0.05),治疗后2组间比较差异有统计学意义(P<0.05)。结论:正本清源汤联合米非司酮治疗围绝经期功能性子宫出血疗效显著,能有效止血、改善患者的激素水平,进而减少子宫内膜的厚度。

Objective： To survey clinical effects of Zheng Ben Qing Yuan decoction combined with mifepristone in treating dysfunctional uterine bleeding during peri-menopausal period and its effects on endometrial thickness.Methods： Ninety patients were randomized into the observation group and the control group, 45 cases each group.The patients in the observation group took Zheng Ben Qing Yuan decoction and mifepristone at the fifth day after uterine curettage, and the ones in the control group were treated by mifepristone at the fifth day after uterine curettage, both groups were treated for three menstrual cycles, endometrial thickness, the levels of estradiol（E2）, luteinizing hormone（LH） and follicule-stimulating hormone（FSH）, complete hemostasis time and the time of controlling hemorrhage were compared between both groups. Results： The observation group was shorter than the control group in complete hemostasis time and the time of controlling hemorrhage, and the difference had statistical meaning（P〈0.05）. The levels of FSH, LH, E2 after treating were lower than these before treating notably, and the difference showed statistical meaning within two groups before and after treating; the comparisons of the levels of E2 after treating between both groups had statistical meaning（P〈0.05）. Endometrial thickness after treating in both groups was smaller than the thickness before treating, and the difference was statistically significant（P〈0.05）, and the difference showed statistical meaning in the comparisons between both groups after treating（P〈0.05）. Conclusion： Zheng Ben Qing Yuan decoction combined with mifepristone is effective in treating dysfunctional uterine bleeding during peri-menopausal period,which could effectively stop the bleeding, improve hormone levels of the patients, therefore to reduce endometrial thickness.
