摘要 目的:分析我院结核病住院患者的耐药状况,为预防治疗结核病和临床用药提供科学依据。方法:收集2014年1月-2017年10月我院收治的625例痰培养阳性肺结核患者的临床资料,并进行一线、二线抗结核药物(异烟肼、乙胺丁醇、利福平、链霉素、左氧氟沙星、卡那霉素、乙硫异烟胺、对氨基水杨酸、阿米卡星、卷曲霉素)的药物敏感度试验。结果:10种抗结核药物耐药率从高到低依次为链霉素36.50%,异烟肼36.17%,利福平26.35%,乙胺丁醇24.30AA,左氧氟沙星21.28%,对氨基水杨酸17.84%,卡那霉素15.71%,卷曲霉素13.26%,阿米卡星12.27%,乙硫异烟胺9.66%;初治患者多耐药率、耐多药率均低于复治患者,差异均有统计学意义(P〈0.05),初治患者、复治患者单耐药率差异无统计学意义(P〉0.05)。结论:我院结核病住院患者耐药水平仍较高,应加强用药管理,依据结核耐药情况进行治疗。 Objective: to To analyze the drug resistance status of tuberculosis (TB) inpatients in our hospital, so as to provide scientific basis for the prevention and treatment of" tuberculosis and clinical medication. Methods: The clinical data of 625 pulmonary tuberculosis patients with 625 positive cases of'sputum culture results from January 2014 to October 2017 in our department were collectedpositive clinical data of patients with pulmonary tuberculosis, and the first-line and second-line anti -tuberculosis drugs (ie,isoniazid, rifampicin" ethambutol, streptomycin, kanamycin" levofloxacin" ethionamide, salicylic acid, Amikacin" capreomycin) drug sensitivity test were performed. Results: The resistance rates of'10 kinds of anti anti-tuberculosis drug resistance rate from high to low were as follows: 36.50% streptomycin 36.50%, isoniazid and rifampicin ethambuto136.17%, rif'ampicin 26.35%, ethambuto124.39%, levofloxacin 21.28%, salicylic acid 17.84%, 15.7 1% kanamycin 15.71%, capreomycin 3.26%, Amikacin 12.27%, ethionamide 9.66%. The ; multi drug resistance rate of initial treatment of patients with initial treatment multi drag resistance rate, the multi drug resistance rate was lower than the retreatment patients, the differences weasre statistically significant (P〈0.05) . The, single drug resistance rate of patients withwith initial treatment and retreatment patients of single drug resistance rate showed no significant difference (P〉0.05) . Conclusion: the The drug resistance level ofTB inpatients in our hospital is still high, so the drug administration should be strengthened and the drug resistance of tuberculosis should be treated according to the drug resistance status.
