摘要
目的探讨首发精神分裂症患者血清脑源性营养因子与脂肪细胞因子及认知功能相关性。方法选取本院2015年1月~2017年1月收治82例首发精神分裂症患者作为首发组、80例停药>30d复发精神分裂症患者作为复发组和80例本院家属健康自愿者作为对照组。测定和对比3组患者血清脑源性神经营养因子(BDNF)、血清脂联素(APN)、激素结合蛋白(SHBG)、瘦素(LEP)、视黄醇结合蛋白4(RBP4)和认知功能。分析首发精神分裂症患者BDNF与脂肪细胞因子及认知功能相关性。结果首发组和复发组研究对象血清BDNF和APN水平显著低于对照组(P<0.05),LEP、SHBG和RBP4水平显著高于对照组(P<0.05)。首发组和复发组研究对象WMS-IV、WAIS-IV各项评分均显著低于对照组(P<0.05)。首发精神分裂症患者血清BDNF水平与APN呈现正相关性(P<0.05),与LEP、SHBG和RBP4均呈现负相关性(P<0.05),首发精神分裂症患者血清BNDF水平与各项认知功能呈现正相关性(P<0.05)。结论首发精神分裂症患者血清BDNF水平与血清脂肪因子水平及认知功能具有相关性,低表达BNDF可能是首发或复发精神分裂症的危险因素。
Objective To investigate the relationship between serum brain derived neurotrophic factor (BDNF), cytokines and cognitive function in patients with first-episode schizophrenia. Methods A total of 82 cases of firstepisode schizophrenia patients were selected as the first-episode group, while 80 patients with drug withdrawal 〉30d recurrence of schizophrenia were selected as the recrudescence group and 80 cases of healthy families volunteers were selected as the control group in our hospital from January 2015 to January 2017. Serum brain-derived neurotrophic factor (BDNF), serum adiponeetin (APN), hormone binding protein (SHBG), leptin (LEP), retinol binding protein 4 (RBP4) and cognitive function were measured and compared among the 3 groups. To analyze the association of BDNF with fat cell factor and cognitive function in first-episode schizophrenia. Results The serum levels of BDNF and APN in the first-episode group and the recrudescence group were significantly lower than those in the control group (P 〈 0.05 ), and the levels of LEP, SHBG and RBP4 were significantly higher than those of the control group (P 〈 0.05 ) . The scores of WMS-IV and WAIS-IV in the first-episode group and the recrudescence group were significantly lower than those in the control group (P 〈 0.05 ) The serum BNDF levels of the first-episode schizophrenia patients showed positive correlation with APN (P〈0.05), and showed negative correlations with LEP, SHBG and RBP4 (P〈0.05). Serum BNDF level in showed positive correlation with cognitive function scores the first-episode schizophrenia patients (P 〈 0.05 ) . Conclusion Serum BNDF levels may have some correlations with serum lipid levels and cognitive function in patients with first-episode schizophrenia, and low expression of BNDF may be a risk factor of first-episode schizophrenia or recrudescence schizophrenia.
