喹硫平与阿立哌唑治疗伴发高血压精神分裂症患者的临床疗效及安全性比较

A control study on the efficacy and safety of quetiapine and aripiprazole in treatment of schizophrenia patients with hypertension

目的对比喹硫平与阿立哌唑治疗伴发高血压的精神分裂症(SCH)患者的疗效及安全性。方法选取伴发高血压的SCH患者92例随机分为A组与B组,每组46例。A组与B组在常规降压治疗基础上分别予以喹硫平和阿立哌唑治疗,比较两组治疗前后阳性症状和阴性症状量表(PANSS)评分、24h平均收缩压(24hSBP)、24h平均舒张压(24h DBP)、24h收缩压变异(24hSSD)及24h舒张压变异(24hDSD),比较副反应发生情况。结果 A组和B组的总有效率比较差异无统计学意义(93.48%vs 91.30%,P>0.05);治疗8周后,两组24h SBP、24hDBP比较差异无统计学意义(P>0.05),但A组的24hSSD、24h DSD均显著低于B组(P<0.05);治疗后两组PANSS总分及各因子评分均显著降低(P<0.05),但组间比较差异无统计学意义(P>0.05);A组副反应发生率较B组显著降低(28.26%vs 56.52%,P<0.05)。结论喹硫平与阿立哌唑治疗伴高血压SCH患者均具有肯定疗效,但喹硫平具有更好的辅助降压作用且副反应更少。

Objective To compare the efficacy and safety of quetiapine and aripiprazole in treatment of schizo- phrenia （ SCH ） patients with hypertensive. Methods 92 SCH patients with hypertensive were randomly divided into group A and B group, 46 cases in each group. On the basis of conventional antihypertensive therapy, patients in group A and B received quetiapine and aripiprazole respectively. The score of （PANSS）, levels of 24h average systolic blood pressure （ 24h SBP）, 24h average diastolic blood pressure （ 24h DBP ）, 24h systolic pressure variation （ 24h SSD ） and 24h diastolic blood pressure variation （ 24hDSD ） were compared in the two group before and after treatment, the drug adverse reaction were compared. Results There was no significant deference of the total effective rate between group A and B （ , 93.48 % vs 91.30%, P〉0.05 ）; 8 weeks after treatment, the levels of 24hSBP and 24DBP between the two groups were no significant deferences （P 〉 0.05 ）, but the 24hSSD and 24hDSD in group A were significantly lower than those in the group B （P 〈 0.05 ） . After treatment, the scores of PANSS and each factor were significantly decreased （P 〈 0.05 ）, but there were no significant differences between the two groups （P 〈 0.05 ） ; The incidence of drug adverse reaction in group A was significantly lower than that in group B （28.26 % vs 56.52 %, P 〈 0.05 ） . Conclusion Que- tiapine and aripiprazole are both effective in the treatment of SCH patients with hypertensive, but quetiapine has better effect for auxiliary blood pressure and less side effects.
