结构-过程-结果三维质量评价模式在手术室感染控制中的应用效果

Application Effect of Structure-Process-Result 3D Quality Evaluation System on Operating Room Infection Control

目的探讨结构-过程-结果三维质量评价模式在手术室感染控制中的应用效果。方法便利抽样法选取2015年1-12月某院应用传统常规管理法进行感染控制期间的12 869例手术患者和手术室卫生学监测样本1929件作为对照组;同法选取2016年1-12月某院应用结构-过程-结果模式进行感染控制期间的12 376例手术患者和手术室卫生学监测样本2148件作为观察组。比较两组患者的手术切口感染率和卫生学监测样本的合格率。结果观察组中Ⅰ类手术、Ⅱ类手术的切口感染率和总感染率分别为0.17%、0.35%和0.22%,均低于对照组的0.36%、0.72%和0.47%,差异均有统计学意义(均P<0.05);且观察组除物体表面和消毒液两项指标外,其他各指标卫生学监测合格率和总合格率均高于对照组,差异均有统计学意义(P<0.05)。结论采用结构-过程-结果三维质量评价模式是控制手术室感染的有效手段,建议推广。

Objective To explore the effect of structure-process-result 3D quality evaluation system on infection control in operating room.Methods 12869 cases of surgical patients and 1929 operating room hygiene monitoring samples from January to December 2015 by using conventional management method for operating room infection control were roiled into as the control group, 12 376 cases of surgical patients and 2148 operating room hygiene monitoring samples from January to December 2016 by using structure-process-result 3/） quality evaluation system for operating room infection control were rolled into as the observation group.The surgical inci- sion infection rate and qualified rate of monitoring samples between the two groups were compared.Results The infection rate of class Ⅰ,class Ⅱ surgical incision and the total infection rate in the observation group was 0.17%, 0.35%and 0.22% respectively, which was significantly lower than that in the control group of 0.36 % ,0.72% and 0.47 % respectively （all P〈0.05）.Except two index of sample surface and disinfection fluid,the total qualified rate and qualified rate of rather hygiene monitoring indexes in the observation group was significantly higher than those in the control group.All the differences were statistically significant（P~ 0.05）.Conclusion The structure- process-result 3D quality evaluation system was effective to enhance operating room infection control, which is worth of clinical promotion.
