47例显微血管减压术治疗面肌痉挛临床分析

Clinical Analysis of 47 Cases with Microvascular Decompression to Treat Hemifacial Spasm

目的:探讨显微血管减压术治疗面肌痉挛的手术方法及效果。方法:回顾性分析了47例行显微血管减压术治疗的面肌痉挛病人的临床资料,观察分析其术前影像学资料,术中所见及术后并发症等情况。结果:47例患者中,术中证实压迫血管分别为SCA8例,PICA11例,AICA24例,VA2例,AICA及VA共同压迫2例。术后症状即消失38例,一周内消失1例,2个月内消失8例。术后发生面瘫者6例,其中3例为迟发性。听力减退者3例。结论:显微血管减压术是治疗面肌痉挛的有效方法,对责任血管的判定有助于提高手术效果。

Objective：To analyze the clinical and surgical results of microvascular decompression（MVD） treatment in patients with hemifacial spasm（HFS）. Methods：Forty-seven cases of HFS underwent MVD were analyzed retrospectively.Results：Forty-seven cases（ male 18, female 29） of HFS which neurovascular compression were identified during operation： 8 cases with SCA, 11 cases with PICA, 24 cases with AICA, 2 cases with BA, 2 cases with VA and AICA. HFS disappeared immediately after MVD in 38 cases and 1 case disappeared within 1 week and 8 cases disappeared within 2 months. postoperative complications included： facial palsy in 6 cases, decreased hearing ability in 3 cases. Conclusion：MVD is an effective treatment for HFS. Correct identification of the affected vessel can increase the operative efficacy.
