摘要
目的:探究并分析不同层数CT冠脉成像用于冠脉疾病诊断的效果对比。方法:选取2013年6月~2016年6月于我院行冠脉成像检查(包括64层及128层CT成像检查)的冠心病患者115例,以上患者均在一周内行CT冠脉造影检查以明确诊断。观察并记录下两种CT成像下患者冠脉梗阻情况,比较两种CT冠脉成像诊断方法的准确性(包括敏感性、特异性及一致性)。结果:经秩和检验,两种层数CT成像下LM(左主干)、LAD(左前降支)、LCX(回旋支)、D1-2(前降支起始部至第1/2对角支或间隔支发出处)、OM1-2(钝缘支)成像结果差异明显(u=2.07、1.96、2.52、1.97、2.09、2.08,P<0.05),两种层数CT成像下RCA、PDA、PLVB成像结果差异明显(u=1.97、2.03、2.06、2.06,P<0.05),64层CT冠脉成像敏感性、特异性、一致性分别为81.0%、90.1%、85.6%,128层CT冠脉成像敏感性、特异性、一致性分别为94.8%、96.2%、95.5%,128层CT冠脉成像敏感性和一致性明显高于64层CT成像(P<0.05),具有统计学意义。结论:128层CT冠脉成像较64层冠脉CT成像更能有效提高冠脉系统疾病的诊断率,值得临床推广。
Objective: To investigate and analyze the effect of coronary angiography with different layers of CT in coronary artery disease diagnosis. Methods: from June 2013 to June2016 in our hospital underwent coronary angiography examination(including 64 layer and 128 layer CT imaging) 115 cases of patients with coronary heart disease, the patients were within one week of CT coronary angiography to confirm the diagnosis.The coronary obstruction in the two CT images was obsenved and recorded, and the accuracy of the two CT coronary angiography methods(including sensitivity, specificity, and consistency)was compared. Results: The rank sum test, two layers of CT imaging LM(LM), LAD(left anterior descending), LCX(LCX),D1-2(anterior descending to the origin of the 1/2 diagonal branch or interval Branch source), OM1-2(obtuse marginal branch) imaging results significantly different(u=2.07, 1.962.52, 1.97, 2.09, 2.08, P〈0.05), two layers of RCA, PDA, CT imaging PLVB imaging results significantly different(u=1.97,2.03, 2.06, 2.06, P〈0.05), 64 slice CT coronary artery imaging sensitivity, specificity and consistency were 81%, 90.1%,85.6%, 128 slice CT coronary artery imaging the sensitivity,specificity and consistency were 94.8%, 96.2%, 95.5%, 128 layers of CT coronary artery imaging sensitivity and consistency was significantly higher than that of the 64 layer CT imaging(P〈0.05), with statistical significance.Conclusion: the 128 slice CT coronary angiography is more effective than the 64 slice coronary CT imaging in improving the diagnostic rate of coronary artery disease. It is worthy of clinical promotion.
