2013-2015年瑞丽市边境地区不同档次缅籍暗娼HIV/梅毒感染及相关行为分析

[目的]分析瑞丽市边境地区不同档次娱乐场所缅籍暗娼人群现况、行为特征、干预服务及艾滋病病毒(HIV)/梅毒感染状况,为制定边境口岸缅籍暗娼的HIV干预策略提供依据。[方法]于2013-2015年,对瑞丽市边境地区不同档次娱乐场所缅籍暗娼人群开展问卷调查及血清学监测,使用SPSS 19.0软件分析。[结果]2013-2015年共调查缅籍暗娼965人,各年分别是336人、280人和349人。其中低档307人(31.8%)、中档420人(43.5%)和高档238人(24.7%)。2003-2015年艾滋病知识知晓率分别为97.3%、99.6和98.9%。最近一次商业性行为安全套坚持使用率分别为82.1%、95.4%和99.1%。最近一个月商业性行为安全套坚持使用率分别为74.4%、92.5%和95.1%。吸食毒品比例分别为4.8%、1.1%和0.9%。至少接受过一次干预比例分别为77.1%、98.2%和95.7%。最近一年检测过艾滋病并知晓结果比例分别为52.1%、88.6%和84.8%。HIV检出率各年分别为4.5%、3.2%和1.4%。梅毒确认阳性检出率分别为1.2%、1.8%和0%。[结论]2013-2015年瑞丽市边境地区娱乐场所缅籍暗娼艾滋病知识知晓率、商业性行为安全套坚持使用率、接受过HIV检测并知晓结果比例逐年上升,并保持在较高水平;吸食毒品比例、艾滋病检出率、梅毒检出率呈现下降趋势;但缅籍暗娼尤其是缅籍低档暗娼HIV/梅毒感染率依然较高,高危行为的风险较大。

Objective To analyze current status,behavioral characteristics,intervention services and HIV/syphilis infection among Burmese unlicensed prostitute in different level entertainment venues in border of Ruili so as to provide basis for formulating HIV intervention strategies for Burmese unlicensed prostitutes at the border ports. Methods From 2013 to 2015,questionnaire survey and serological surveillance were conducted for Burmese unlicensed prostitutes in entertainment venues of different grades in Ruili. SPSS 19. 0 software was used for analysis. Results From 2013 to 2015,total of 965 Burmese unlicensed prostitutes were investigated,with 336 280 and 349 respectively in each year. Among them,307（ 31. 8%） worked in low-end venues,420（ 43. 5%） were in middle-end venues and 238（ 24. 7%） were in high-end venues. The awareness rate of AIDS was 97. 3%,99. 6% and 98. 9% respectively over the three years. The percentage of consistent use of condoms during the last commercial sexual behavior was 82. 1%,95. 4% and 99. 1% in each year. The percentage of consistent use of condoms during the commercial sexual behaviors in the last month was 74. 4%,92. 5% and 95. 1% in each year. The rate of drug abuse was 4. 8%,1. 1% and 0. 9% respectively. The rate of receiving least one intervention was 77. 1%,98. 2% and 95. 7% respectively. The number of people among them who received the test for AIDS and were aware of the results in the most recent year was 52. 1%,88. 6% and 84. 8% respectively.The detection rate of HIV in each year was 4. 5%,3. 2% and 1. 4%. The syphilis positive rate was 1. 2%,1. 8% and 0% respectively. Conclusions From 2013 to 2015,the awareness rate of AIDS prevention knowledge,the usage rate of condoms during commercial sex behaviors,the proportion who had received HIV detection and known the results had risen year by year,and maintained at a relatively high level. The proportion of drug abuse,AIDS detection rate and syphilis detection rate shows downswing. However,the infection rate of HIV/syphilis among Burmese unlicensed prostitutes in entertainment venues,especially the low-end＇ s venues,is still high. High-risk behaviors is still large.
