血清骨桥蛋白、癌胚抗原及CK18的表达对胃癌的诊断价值

Significance of serum osteopontin,CEA and CK18 for gastric cancer diagnosis

目的:测定胃癌患者血清中骨桥蛋白(OPN)、癌胚抗原(CEA)和细胞角蛋白18(CK18)的表达情况,探讨其在胃癌诊断中的意义。方法:选择80例胃癌患者作为胃癌组,80名健康体检者作为对照组,观察胃癌组和对照组血清OPN、CEA和CK18含量,血清OPN和CEA、CK18诊断胃癌的灵敏度、特异度及ROC曲线下面积,血清OPN、CEA和CK18含量和胃癌病理特征的关系。结果:胃癌组患者血清OPN、CEA和CK18含量均高于对照组(P<0.05)。血清OPN诊断胃癌的ROC曲线下面积大于血清CEA和CK18,两两联合指标诊断胃癌的ROC曲线下面积大于单独指标,三者联合诊断胃癌的ROC曲线下面积最大。随着联合指标的增多,诊断胃癌的灵敏度升高。结论:胃癌患者血清OPN、CEA和CK18水平升高,多指标联合检测诊断胃癌的灵敏度比较高。

Aim：To detect the value of the expressions of serum osteopontin（OPN）,carcinoembryonic antigen（CEA）and cytokeratin18（CK18）in gastric cancer diagnosis.Methods：A total of 80 cases of gastric cancer patients were selected as gastric cancer group and 80 healthy individuals as control group.The contents of serum OPN,CEA and CK18 in the 2 groups were determined,the sensitivity,specificity and area under the ROC curve of serum OPN and CEA,CK18 contents in diagnosis of gastric cancer were caculated,and the relationship between serum OPN,CEA and CK18 contents and pathological features of gastric cancer was analyzed.Results：The contents of serum OPN,CEA and CK18 in gastric cancer group were higher than those in control group（P〈0.05）.The area under the ROC curve of serum OPN was larger than serum CEA and CK18 in diagnosis of gastric cancer,the two-two joint indexes in diagnosis of gastric cancer had the largert area under the ROC curve than individual indicator,and the three indicators joint had the largest area under the ROC curve.With the joint index increasing,the diagnosis sensitivity of gastric cancer increased.Conclusion：The contents of serum OPN,CEA and CK18 of patient with gastric cancer are elevated,and the sensitivity of multiple indicators in diagnosis of gastric cancer is relatively high.
