摘要
目的探讨血清CA125、CA199、AFP和CEA在卵巢癌中的表达及联合诊断价值,为卵巢癌的早期诊断提供临床依据。方法选取该院和肿瘤医院2016年1月-2017年6月收治的50例卵巢癌患者(卵巢癌组)、60例卵巢良性肿瘤患者(良性肿瘤组)为研究对象,同时以60例妇科良性疾病患者为对照,设为对照组。对血清CA125、CA199、AFP和CEA水平进行检测,比较3组CA125、CA199、AFP和CEA的表达水平,并对各指标的单一及联合检测阳性率进行对比分析,同时进一步对卵巢癌不同分期中4项指标单一及联检阳性率进行对比分析。结果卵巢癌组血清CA125、CA199、AFP和CEA检测水平均明显高于良性肿瘤组,比较差异具有统计学意义(P<0.05),良性肿瘤组患者CA125、CA199、AFP和CEA表达与对照组比较差异无统计学意义(P>0.05)。在4项指标中,CA125拥有最高的诊断阳性率,4项指标联检的阳性率显著高于各单一指标阳性率,差异具有统计学意义(P<0.05)。卵巢癌不同分期中单一指标阳性率最高者为CA125,4项指标联检阳性率要明显高于单一指标阳性率,且各单一指标及联合检测阳性率随着卵巢癌分期的增加而逐渐升高。结论在卵巢癌的诊断中,CA125拥有较高的阳性检出率,而CA125、CA199、AFP和CEA 4项联检可以获得更高的诊断准确率,其对于卵巢癌早期诊断具有重要意义。
Objective To explore the expressions of serum CA125,CA199,AFP,and CEA in ovarian cancer and the value of combined diagnosis,provide a clinical basis for early diagnosis of ovarian cancer. Methods From January 2016 to June 2017,50 patients with ovarian cancer( ovarian cancer group) and 60 patients with benign ovarian tumor( benign tumor group) were selected from the Second Affiliated Hospital of Medical College,Shantou University and Tumor Hospital as research objects,and 60 patients with benign gynecological diseases were selected as control group. The levels of serum CA125,CA199,AFP,and CEA were detected. The expression levels of CA125,CA199,AFP,and CEA in the three groups were compared. The positive rates of single detection and combined detection of the above-mentioned indexes were compared and analyzed. In ovarian cancer group,the positive rates of single detection and combined detection of the above-mentioned indexes were compared among the patients of different stages. Results The levels of serum CA125,CA199,AFP,and CEA in ovarian cancer group were statistically significantly higher than those in benign tumor group( P〈0. 05). There was no statistically significant difference in the expression levels of CA125,CA199,AFP,and CEA between benign tumor group and control group( P〈0. 05).Among the four indexes,the positive diagnostic rate of CA125 was the highest,the positive rate of combined detection of the four indexes was statistically significantly higher than those of single detection( P〈0. 05). In ovarian cancer group,the positive rate of CA125 was the highest among the patients of different stages,the positive rate of combined detection of the four indexes was statistically significantly higher than those of single detection,the positive rates of combined detection and single detection increased gradually with the increase of staging of ovarian cancer. Conclusion Among the diagnostic indexes of ovarian cancer,the positive detection rate of CA125 is high,but the combined detection of CA125,CA199,AFP,and CEA can obtain higher accurate rate,which has important significance for early diagnosis of ovarian cancer.
