探究PFNA内固定和人工股骨头置换术在高龄股骨粗隆间骨折治疗中的运用价值

To explore the value of PFNA internal fixation and artificial femoral head replacement in the treatment of intertrochanteric fractures of the elderly

对PFNA内固定与人工股骨头置换术在高龄股骨粗隆间骨折治疗中的临床运用价值进行研究。方法：抽取2015年3月-2017年3月我院所收治的高龄股骨粗隆间骨折患者作为研究对象，按手术方法不同分成观察组与对照组，观察纽采取PFNA内固定术治疗，对照组患者予以人工股骨头置换术治疗；比较2组患者的手术治疗情况。结果：观察组患者总有效率89．7％，高于对照组60．O％，差异有统计学意义（X^2=5．228，P〈0．05）；观察纽患者术后1个月、术后2个月的Harris评分均低于对照组，差异显著，具备统计学意义（P〈0．05）；观察组患者住院时间多于对照组，差异有统计学意义（P〈0．05）；观察组患者并发症发生率（5．2％）低于对照组（40．O％），差异显著（X^2=4．326，P〈0．05）。结论：对高龄股骨粗隆间骨折患者予以PFNA内固定术治疗，效果显著．且并发症少，值得大力推广。

Objective To study the clinical value of PFNA internal fixation and artificial femoral head replacement in the treatment of intertrochanteric fractures of the elderly.Methods From March 2015 to March 2017,the patients with intertrochanteric fractures were di-vided into fixed group and replacement group.The fixed group was treated with PFNA internal fixation,and the patients in the replace-ment group Artificial femoral head replacement surgery;comparison of the two groups of patients with surgical treatment and.Results The total effective rate（89.7%）in the fixed group was higher than that in the replacement group（60.0%）,the difference was statistically significant（x2 = 5.228,P 〈 0.05）;fixed group of patients after 1 month,2 months after surgery（ P 〈 0.05）.The incidence of com-plications in the fixed group was significantly higher than that in the replacement group（ P 〈 0.05）.The difference was statistically sig-nificant（ P 〈0.05）5.2%）was lower than that of the replacement group（40.0%）,the difference was significant（ X^2 = 4.326,P 〈 O.05）.Conclusion PFNA internal fixation is effective in patients with intertrochanteric fractures of the elderly,and the effect is significant and the complications are less.
