泰安某社区2型糖尿病患者脂质蓄积指数与缺血性心血管病发病风险的相关性研究

Study on Correlation Between Lipid Accumulation Product and the Risk of Ischemic Cardiovascular Disease of Type 2 Diabetes in a Community of Tai'an City

目的探讨脂质蓄积指数(LAP)与2型糖尿病患者缺血性心血管病(ICVD)发病风险的相关性,为2型糖尿病患者改变生活方式提供一定指导。方法选取泰安市某社区2型糖尿病患者316例为研究对象。检测其身高、体重、腰围、血糖、血压、血脂等相关指标,并计算BMI、LAP和ICVD发病风险。应用Spearman相关模型分析LAP与心血管病危险因素的关系;应用线性回归模型分析LAP与ICVD发病风险的相关性,P<0.05为差异有统计学意义。结果随着空腹血糖的升高,LAP和ICVD发病风险均显著增加(均P<0.05)。LAP与FPG、收缩压、舒张压、TC、LDL-C呈正相关(P<0.05),与HDL-C水平呈负相关(P<0.05),与年龄、TC无相关关系(P>0.05)。多因素线性回归分析显示:性别、年龄、吸烟、收缩压、TC和LAP可解释ICVD发病风险的61.5%。结论 LAP升高会伴随较高的心血管危险因素水平。在一定程度上,可作为筛查2型糖尿病患者中ICVD发病的指标。

Objective To explore the correlation between lipid accumulation product（LAP）and risk of ischemic cardiovascular disease（ICVD）,and provide guidance for changing the lifestyle of patients with type 2 diabetes mellitus（T2 DM）.Methods A total of 316 patients with T2 DM were selected in a community of Tai＇an city.The height,weight,waist circumference,blood glucose,blood pressure,blood lipids were measured,and the risk of BMI,LAP and ICVD was calculated.The relationship between LAP and the risk factors of cardiovascular was analyzed by Spearman correlation model.Linear regression model was used to analyze the association between LAP and ICVD.Results With the increase of fasting blood glucose,LAP and the risk of ICVD were increased significantly（P〈0.05）.LAP was positively correlated with FPG,systolic blood pressure,diastolic blood pressure,TC and LDL-C（P〈0.05）,and negatively correlated with HDL-C level（P〈0.05）,but not related to age and TC（P〉0.05）.Multiple factors linear regression analysis indicated that gender,age,smoking,systolic blood pressure,TC,and LAP can explain 61.5% of the risk of ICVD.Conclusion Higher LAP levels are associated with higher levels of cardiovascular risk factors.To a certain extent,it can be used as an indicator of the incidence of ICVD in patients with type 2 diabetes.
