通窍活血汤对脑梗死患者血流动力学及血液流变学的影响

Effect of Tongqiao Huoxue decoction in the treatment of cerebral infarction and the influence on hemodynamics and hemorheology

目的研究通窍活血汤治疗脑梗死的临床效果和对患者血流动力学、血液流变学的影响。方法选择2014年6月-2017年6月在我院进行诊治的脑梗死患者86例,随机分为2组,各43例。2组均采用常规的西医治疗,对照组口服安慰剂,观察组口服中药通窍活血汤,比较2组的临床治疗效果,治疗前后的血清白介素-6和C反应蛋白水平,血小板聚集率和血粘稠度,并观察2组双侧大脑的最大峰值流速、血管阻力指数、平均流速以及搏动指数等血流动力学指标。结果观察组总有效率90.70%,明显高于对照组的74.42%(P<0.05);治疗后2组血黏稠度、血小板聚集率、血清白介素-6和C反应蛋白水平均明显降低(P<0.05),且观察组降低更为明显(P<0.05);治疗后2组血管阻力指数以及搏动指数均明显降低(P<0.05),最大峰值流速以及平均流速均明显升高(P<0.05),且观察组更为明显(P<0.05)。结论通窍活血汤对脑梗死具有明显疗效,可有效改善患者血流动力学及血液流变学,减轻炎性反应损伤程度。

Objective To investigate the clinical effect of Tongqiao Huoxue decoction in the treatment of cerebral infarction and the influence on hemodynamics and hemorheology.Methods Selected 86 cases of patients with cerebral infarction who were treated in our hospital from June 2014 to June 2017,divided into two groups randomly.Two groups were treated with conventional western medicine treatment,the control group received placebo.The observation group was treated with Tongqiao Huoxue decoction.Compared with the clinical curative effect,the level of serum before and after treatment of-6 and C reactive protein,platelet aggregation and blood viscosity,and to observe the 2 groups of bilateral brain peak velocity,mean velocity,vascular resistance index and pulsatility index and hemodynamics.Results After treatment,the effective rate of the observation group was significantly higher than the control group（P〈0.05）;blood viscosity,platelet aggregation rate,serum interleukin-6 and C reactive protein levels in the 2 groups were significantly decreased（P〈0.05）,and the decrease in the observation group was more significant（P〈0.05）.The vascular resistance index and pulsatility index of the 2 groups were significantly decreased（P〈0.05）,and the maximum peak velocity and mean velocity were significantly increased（P〈0.05）,and the observation group was more significant（P〈0.05）.Conclusion Tongqiao Huoxue decoction has remarkable curative effect on cerebral infarction can effectively improve hemodynamics and blood rheology,alleviate inflammatory injury degree.
