中药熏蒸联合穴位按摩在髋部骨折患者术后康复中的应用

Effect of traditional Chinese medicine fumigation and acupoint massage on postoperative rehabilitation of hip fracture patients

目的探讨自拟中药方熏蒸联合穴位按摩在髋部骨折患者术后康复中的应用效果。方法选取2014年1月-2015年1月在云南省第三人民医院骨科拟行人工髋关节置换术的80例老年股骨颈骨折患者作为研究对象,按照随机数字表方法分为观察组(n=40)和对照组(n=40),对照组予术后常规对症治疗,观察组在对照组的基础上实施自拟中药方熏蒸联合穴位按摩进行治疗,分别对2组不同时间节点的血浆eNOS与iNOs水平、术后恢复情况、术后并发症评分等相关指标进行客观评定。结果 2组手术前和术后4 d时的eNOS与iNOs水平差异无统计学意义(P>0.05),术后7 d时,观察组eNOS与iNOs水平高于对照组(P<0.05);与对照组相比,观察组患者的住院时间更短,但组间差异没有统计学意义(P>0.05),观察组下床锻炼时间及关节功能恢复情况明显优于对照组(P<0.05);2组并发症发生情况比较,差异有统计学意义(P<0.05)。结论自拟中药方熏蒸联合穴位按摩有助于提高患者eNOS与iNOs水平,促进骨折愈合,并能有效降低术后并发症的发生。

Objective to discuss effect of self-made prescription traditional Chinese medicine and acupoint massage on postoperative rehabilitation of hip fracture patients.Methods from January 2014 to January 2015 in our hospital 80 cases of femoral neck underwent artificial hip replacement fracture as the research object,according to the random number table method divided into observation group（n = 40）and control group（n = 40）,the control group was given routine symptomatic treatment after operation,the observation group was treated with traditional Chinese medicine fumigation and acupoint massage self-made prescription traditional Chinese medicine and acupoint massage.Objective levels of plasma eNOS and iNOs levels,postoperative recovery and postoperative complications were evaluated objectively in two groups of patients at different time points.Results by comparison,no statistically significant difference between eNOS and iNOs levels in two groups of patients before and after surgery at 4 D（P〈0.05）,postoperative 7 d,eNOS and iNOs level of observation group were significantly higher than that of control group（P〈0.05）;compared with the control group,hospitalization time of observation group were shorter,but the difference between the two groups was not statistically significant（P〉0.05）,the observation group of patients with bed time and joint function recovery was significantly better than the control group（P〈0.05）;there was significant difference between the incidence of the two groups of patients with complications（P〈0.05）.Conclusion Steaming and acupoint massage combined with Chinese herbal medicine can improve the level of eNOS and iNOs,promote the healing of fracture,and effectively reduce the incidence of postoperative complications.
