超声引导下经皮穿刺置管引流治疗重症急性胰腺炎的临床疗效

Clinical effect of ultrasound-guided percutaneous catheter drainage in the treatment of severe acute pancreatitis

目的:探讨超声引导下经皮穿刺置管引流在重症急性胰腺炎治疗中的临床疗效。方法:选取我院2015-09-2017-12期间收治的重症急性胰腺炎患者176例进行回顾性分析,随机分为观察组与对照组,对照组行保守治疗,观察组在对照组基础上行超声引导下经皮穿刺置管引流治疗,观察比较两组治疗后治疗效果、相关实验室指标、并发症、中转手术情况和总有效率。结果:观察组治疗后积液消失时间,血清CRP、体温和血淀粉酶恢复正常时间及总住院时间明显低于对照组,实验室检查指标明显优于对照组,严重并发症发生率、中转手术率和总有效率明显优于对照组,差异均具有统计学意义(P<0.05)。结论:运用超声引导下经皮穿刺置管引流方法治疗重症急性胰腺炎,具有有效、微创、方便、安全等优点,值得临床推广应用。

Objective：To investigate the ultrasound guided percutaneous catheter drainage in clinical curative effect in the treatment of severe acute pancreatitis.Method：One hundred and seventy-six patients with severe acute pancreatitis who are treated in our hospital from September 2015 to December 2017 were selected and analyzed retrospectively.They were randomly divided into observation group and control group,control group received conservative treatment,the observation group in the control group on the basis of ultrasound guided percutaneous catheter drainage treatment.After the treatment,treatment effect,laboratory indexes,complications,transit surgery rate and the total efficiency of the two groups were compared.Result：Time of effusion disappearance,time of serum CRP,body temperature and amylase return to normal and total hospitalization time of the observation group were significantly lower than the control group.The laboratory examination index was significantly better than the control group.The incidence of serious complications,the rate of transfer surgery and the total effective rate were significantly better than the control group,with a statistically significant differences（P0.05）.Conclusion：Ultrasound guided percutaneous catheterization and drainage is effective,minimally invasive,convenient and safe in the treatment of severe acute pancreatitis,so it is worth popularizing and applying in clinic.
