摘要
目的研究探讨应用腹腔镜修补术治疗急性胃穿孔的临床效果。方法选取揭阳市人民医院2015年10月—2017年8月间收治的86例急性胃穿孔患者为研究对象,并将其分为研究组与对照组,每组各43例,给予对照组传统开腹手术治疗,研究组则应用腹腔镜修补术治疗。观察并比较两组患者术中和术后情况、临床治疗效果以及并发症发生情况。结果研究组手术时间、术后肛门排气时间、住院时间、胃肠功能恢复时间均明显低于对照组,且术中出血量少于对照组,差异有统计学意义(P<0.05);治疗总有效率是95.35%,比对照组的79.07%高,并发症发生率为4.65%,明显比对照组23.26%低,两组间比较,差异有统计学意义(P<0.05)。结论应用腹腔镜修补术治疗急性胃穿孔具有良好的临床效果,能有效减少手术时间及术中出血量,创伤小、患者术后恢复快,且并发症较少,安全性较高,值得临床推广与应用。
Objective To investigate the clinical effect of treatment of acute gastric perforation by laparoscopic repairment.Methods 86 cases with acute gastric perforation from October,2015 to August,2017 in the hospital were regarded as the research object,and were divided into study group and control group,with 43 cases in each group.The control group received traditional open surgery treatment,and study group received laparoscopic repair in the treatment of observation and neutralization.Intra-operative and post-operative condition,incidence of clinical therapeutic effect and complications of two groups were compared.Results The operation time,anal exhaust time,postoperative hospitalization time,and gastrointestinal function recovery time are significantly lower than the control group,and intraoperative bleeding is less than the control group,with significant difference(P〈0.05).The total efficiency of treatment is 95.35%,which is higher than 79.07% in control group.The complication rate is 4.65%,which is obviously lower than 23.26% in control group.The difference between the two groups is significant(P〈0.05).Conclusion The application of laparoscopic repair in the treatment of acute gastric perforation has good clinical effect that can effectively reduce the amount of bleeding,operation time and less trauma,faster postoperative recovery,less complications and high safety.It is worthy of clinical popularization and application.
