牙源性黏液瘤及牙源性纤维黏液瘤病例分析

Analysis of Cases with Odontogenic Myxoma or Odontogenic Fibromyxoma

目的分析牙源性黏液瘤及牙源性纤维黏液瘤病例特点,为临床提供资料。方法对病理诊断为牙源性黏液瘤8例和牙源性纤维黏液瘤2例的临床表现、X线、病理特征及治疗进行总结。结果 10例患者均呈无痛性膨胀性生长,常有局部浸润性;X线呈圆形或椭圆形透射区,外周轮廓不清晰或不规则;肉眼呈淡蓝、白色胶冻样,或黄、灰白色质软。镜下丰富的黏液质,其中瘤细胞呈星形或纺锤形,牙源性纤维黏液瘤所含纤维成分多于牙源性黏液瘤;采用局部扩大切除,均无复发。结论牙源性黏液瘤及牙源性纤维黏液瘤是同一类颌骨良性肿瘤,有明显局部浸润性,手术扩大切除效果好。

Objective To analyze the clinical characteristics of odontogenic myxomas and odontogenic ibromyxoma.Methods8 cases with odontogenic myxoma and 2 cases with odontogenic fibromyxoma were analyzed in terms of clinical data,radiographic images,histopathology and treatment.Results The usual clinical presentation is a painless swelling,with local invasive.Radiographs shows round or oval radiolucency.The outer contour is not clear or irregular.Under the naked eyes,the gelatin sample is pale blue,white,or yellow gray soft.Under the microscope,the tumor is composed of a pale basophilic mucoid stroma with a loose arrangement of spindle,or stellate-shaped cells.The amount of collagen of odontogenic fibromyxoma are more than odontogenic myxoma＇s.There is no recurrence after extended local excision.Conclusion Dontogenic myxomas and odontogenic fibromyxoma is the same type of jaw benign tumor,with obvious local invasive characteristics.Extended local excision is an effective method.
