阿昔洛韦联合喜炎平在治疗小儿传染性单核细胞增多症中的临床疗效观察

Clinical observation of acyclovir combined with Xiyanping in the treatment of infectious mononucleosis in children

目的探讨阿昔洛韦联合喜炎平在治疗小儿传染性单核细胞增多症中的临床疗效。方法将参与本次研究的60例患儿,按照其临床症状和性别进行分层抽样,分为治疗组和观察组。治疗组给予患儿喜炎平注射液和阿昔洛韦,观察组患儿仅给予阿昔洛韦,比较两组患儿临床症状(发热),体征(皮疹、淋巴结肿大、咽峡炎、肝脾肿大)消退时间的差异。同时,治疗结束后,对两组患儿分别进行血常规检测,分析其血白细胞、异型淋巴细胞、淋巴细胞比例与观察组治疗后的指标的差异。结果治疗组患儿和观察组患儿热退时间(t=-4.24,P<0.05),咽峡炎消退时间(t=-2.69,P=0.02),淋巴结肿消退时间(t=-2.35,P=0.02),肝脾肿大消退时间(t=-2.71,P=0.01)的差异具有统计学意义。治疗结束后,观察组和治疗组白细胞计数(t=-1.40,P=0.20)、淋巴细胞比例(t=-1.10,P=0.30)、异型淋巴细胞比例(t=-1.80,P=0.10)的差异无统计学意义。结论阿昔洛韦联合喜炎平在治疗小儿传染性单核细胞增多症具有积极作用,建议在临床工作中推广。

Objective To investigate the clinical efficacy of acyclovir combined with Xiyanping in the treatment of infectious mononucleosis in children. Methods Sixty children with stratified sampling according to its clinical symptoms and sex who participated in this study were divided into treatment group and observation group. The treatment group was given Xiyanping injection and Acyclovir, while the observation group was given Acyclovir only. The differences of clinical symptoms（fever）, signs（rash, lymph node enlargement, angina and hepatomegaly） between the two groups were compared. At the same time, after treatment, blood routine test was performed in two groups of children, and the difference of white blood cell, heterotypic lymphocyte and lymphocyte proportion was analyzed and compared. Results There was significant difference between the two groups in the time of heat withdrawal（t=-4.24,P 0.05）, the time of angina regression（t=-2.69,P=0.02）, the time of lymph node regression（t=-2.35,P=0.02） and the regression time of hepatomegaly and splenomegaly（t=-2.71, P=0.01）. After the treatment, there was no statistically significant difference in the white blood cell counts（t=-1.4, P=0.2）, the proportion of lymphocyte（t=-1.1, P=0.3） and the proportion of hetero lymphocyte（t=-1.8, P=0.1） between the observation group and the treatment group. Conclusion Acyclovir combined with Xiyanping has a positive effect on the treatment of infectious mononucleosis in children, and it is recommended to be popularized in clinical work.
