腹腔镜手术与传统开腹手术治疗胃十二指肠溃疡穿孔的效果研究

Effect of laparoscopic surgery and traditional laparotomy on perforation of gastroduodenal ulcer

目的研究比较腹腔镜手术与传统开腹手术治疗胃十二指肠溃疡穿孔的效果。方法随机选取前来我院就诊的胃十二指肠溃疡穿孔患者38例(2016年3月~2017年5月),按照随机数字表法将所有患者分成研究组和对照组,将腹腔镜手术方案和开腹手术方案分别用于两组患者临床治疗,对比分析不同组别患者手术相关指标、术后恢复及并发症发生率等。结果研究组患者手术时间、术后排气时间及住院时间均明显短于对照组,术中出血量明显低于对照组,差异有统计学意义(P<0.05);研究组患者术后疼痛评分明显低于对照组、术后肠蠕动恢复时间明显短于对照组,差异有统计学意义(P<0.05);研究组患者术后并发症发生率明显低于对照组,差异有统计学意义(P<0.05)。研究组手术效果更优。结论胃十二指肠溃疡穿孔患者应用腹腔镜手术治疗,该手术方案安全性更高、术后副作用发生情况控制更好,预后质量更优,临床应用价值突出。

Objective To compare the efficacy of laparoscopic surgery and traditional open surgery in the treatment of perforation of gastroduodenal ulcer. Methods Thirty-eight patients with gastroduodenal ulcer perforation（from March 2016 to May 2017） were randomly selected and divided into study group and control group.Laparoscopic surgery and open surgery were used in clinical treatment of two groups of patients respectively.The related indexes,postoperative recovery and incidence of complications were compared and analyzed in different groups of patients. Results The operative time,postoperative exhaust time and hospitalization time of the patients in the study group were significantly shorter than those in the control group,and the amount of intraoperative bleeding was significantly lower than that in the control group（P 0.05）.The postoperative pain score of the patients in the study group was significantly lower than that in the control group,and the recovery time of postoperative intestinal peristalsis was significantly shorter than that in the control group（P 0.05）.The incidence of postoperative complications in the study group was significantly lower than that in the control group（P 0.05）.The effect of the study group was better.Conclusion Laparoscopic surgery is more safe,side effect is controlled better,prognosis quality is better,clinical application value is outstanding in patients with gastroduodenal ulcer perforation.
