关节镜手术结合早期锻炼干预对膝骨性关节炎患者术后关节功能的影响及疗效

The curative effect and influence of arthroscopic operation combined with early exercise intervention on joint function of patients with knee osteoarthritis

目的探讨关节镜手术结合早期锻炼干预对膝骨性关节炎(KOA)患者术后关节功能的影响及疗效。方法选取2014年1月~2017年2月拟行关节镜手术治疗的70例KOA病例,随机分为干预组和对照组。两组予行关节镜下膝关节探查清理术。对照组予以常规干预,干预组在对照组基础上结合早期锻炼干预。观察两组术前和术后膝关节屈曲度、疼痛评分及膝关节功能,并比较术后临床效果。结果术后4周,两组膝关节屈曲度、疼痛评分及膝关节功能较前改善(P<0.05或P<0.01),且干预组改善情况优于对照组(P<0.05);同时干预组临床总有效率为94.29%,优于对照组的77.14%(χ~2=4.20,P<0.05)。结论关节镜手术结合早期锻炼干预用于KOA术后患者的临床效果较肯定,可增加膝关节屈曲度,减轻疼痛,改善膝关节功能。

Objective To investigate the effect and curative efficacy of arthroscopic operation combined with early exercise intervention on postoperative joint function in patients with knee osteoarthritis（KOA）. Methods 70 patients with KOA who underwent arthroscopic surgery from January 2014 to February 2017 were selected. They were randomly divided into intervention group and control group. Arthroscopic knee probing debridement was performed in both groups. The control group was given routine intervention, and the intervention group was further combined with early exercise intervention on the basis of the control group. The knee joint flexion, pain score and knee joint function of the two groups were observed before and after surgery, and the clinical effect after operation was compared. Results At 4 weeks after surgery, knee joint flexion, pain score and knee joint function were improved in both groups compared with before（P〈0.05 or P〈0.01）, and the improvement in the intervention group was better than that in the control group（P〈0.05）;the total clinical effective rate was 94.29% in the intervention group, which was better than that of 77.14% in the control group（χ^2=4.20, P〈0.05）. Conclusion The clinical effect of arthroscopic surgery combined with early exercise intervention for patients after KOA is favorable, which can increase knee joint flexion, relieve pain and improve knee joint function.
