超声联合MRI检测胎儿脑组织结构畸形与中枢神经系统异常的回顾性研究 被引量:9

摘要 目的:探讨超声联合MRI检测胎儿脑组织结构畸形和中枢神经系统异常情况以及侧脑室增宽与中枢神经异常的相关性,为提高产前诊断率提供依据。方法:以我院2006年2月-2016年12月经超声检查提示有颅脑发育异常且行MRI检查的338例孕妇作为研究对象进行回顾性分析,对超声及MRI检测颅脑异常情况及相符程度进行分析,并对侧脑室增宽与中枢神经异常的相关性进行分析。结果:338例孕妇均接受超声检查并显示颅脑异常,其中147例(43.49%)超声检查结果与MRI检查结果一致,165例(48.82%)经MRI检查后补充诊断,18例经MRI检查后更正诊断,另有8例(2.37%)超声检查颅脑异常在MRI检查中未显示。疑似颅脑异常中侧脑室异常171例(50.59%),小脑延髓池增宽136例(40.24%),其他异常31例(9.17%)。侧脑室异常超声诊断与MRI检查结果分布与小脑延髓池增宽、其他异常情况相比差异具有统计学意义(P<0.05)。其中超声显示双侧侧脑室增宽、侧脑室后角内径>12mm是影响侧脑室增宽合并其他中枢神经系统异常的危险因素(P<0.05)。结论:超声作为孕期检查的常规手段在脑组织结构畸形与中枢神经系统异常中具有较高的检出率,对于侧脑室增宽及其合并其他中枢神经系统异常方面较为突出,通过联合MRI检查侧脑室增宽的部位和后角内径可提高对其是否合并其他中枢神经系统异常的准确率。 Objective :To study the testing of brain tissue deform ity and central nervous system abnormalities in fetaldetected by ultrasonic combined wi th MRI, and provide the basis fo r improving retrospectively analyze 338 cases o f pregnant women wi th craniocerebral dysplasia detected by ultrasound and M R I in our hospital from February 2006 to December 2016, then analyzed the abnormal condition and trasound and MRI, and took correlation analysis of fetal cerebral ventriculomegaly and central nervous abnormality. Re-sults: 338 cases of pregnant women were examined by ultrasound and display the brain abnormalities, there were 147 ca-ses (43. 49%) of the ultrasound results consistent wi th MRI f in d in g s, 165 cases (48. 82%) of supplementary diagnosis after MRI detection, 18 cases of correct diagnosis af ter M R I examination, and another 8 cases normality by ultrasound were not displayed by MR I detection, There were 171 cases (50. 59%) suspected brain abnormalities,136 cases (40. 24%) of posterior cistern extension and 31 cases (9. 17%) of other ab-normity. The differences of comparion lateral ventricle abnormal by ultrasonic diagnosis and ings with posterior cistern extension and other abnormity had sta t is t ica l ly significant difference (P〈0. 05). T he b i la t-eral lateral ventricle broadening detected by ultrasound and posterior horn diameter o f the 12mm were risk factors for fetal cerebral ventriculomegaly combined w i th other central nervous system abnormalities (P 〈0. 05). Conclusion:The ultrasound which was the conventional way of pregnancy check tral nervous system abnormalities and brain s tructure abnormalities, and had more prominent in detecting fetal cerebral ventriculomegaly combined wi th other central nervous system abnormalities, and through combinefetal cerebral ventriculomegaly location and posterior ho rn diameter of the lateral ventriclrate of wheather combined wi th other central nervous system abnormalities.
