关节镜下后内侧入路切除与传统手术切除腘窝囊肿的临床效果对比

Comparison of clinical effect of resection of the posterior medial approach under arthroscopy and traditional surgical resection of popliteal cyst

目的探讨关节镜下后内侧入路切除与传统手术切除腘窝囊肿的临床效果。方法选取2015年3月~2018年2月我院收治的56例腘窝囊肿患者,按照随机数字表法分为对照组和研究组,每组28例。对照组接受传统手术治疗,研究组接受关节镜下后内侧入路手术治疗,观察并比较两组的治疗状况,如手术切口长度、术后感染率、出血量、膝关节功能状况等。结果研究组患者的并发症总发生率低于对照组,差异有统计学意义(P<0.05);研究组患者出血量少于对照组,术后下床活动时间、住院时间均短于对照组,住院费用低于对照组,差异有统计学意义(P<0.05)。两组患者术前Lysholm评分的比较,差异无统计学意义(P>0.05),术后1周,研究组Lysholm评分高于对照组,差异有统计学意义(P<0.05);研究组患者复发0例(0%)少于对照组5例(17.86%),差异有统计学意义(P<0.05);研究组患者的Rauschning分级总有效高于对照组,差异有统计学意义(P<0.05)。结论临床治疗腘窝囊肿患者在满足手术指征的状况下,可将关节镜下后内侧入路切除腘窝囊肿术作为首选方式,其创伤性小,能够促进术后恢复。

Objective To analyze and compare the clinical effects of resection of the posterior medial approach under arthroscopy and traditional surgical resection of popliteal cyst.Methods Fifty-six patients with popliteal cyst admitted to our hospital from March 2015 to April 2018 were randomly divided into control group and study group,with 28 cases in each group.The control group received traditional surgical treatment,while the study group received arthroscopic posterior medial approach,the treatment status of the two groups was observed and compared,such as incision length,postoperative infection rate,bleeding volume,knee function and so on.Results The total incidence of complications in the study group was lower than that in the control group,and the difference was statistically significant（P〈0.05）.The amount of bleeding in the study group was less than that in the control group,the postoperative ambulation time and hospitalization time were shorter than those in the control group,Hospitalization expenses were lower than those in the control group and the differences were statistically significant（P〈0.05）.There was no significant difference in Lysholm score between the two groups before operation（P〉0.05）.One week after operation,Lysholm score in the study group was higher than that in the control group,and the difference was statistically significant.The recurrence of 0 cases（0%） in the study group was lower than that of 5 cases（17.86%） in the control group,and the difference was statistically significant（P〈0.05）.The Rauschning grade in the study group was significantly higher than that in the control group,and the difference was statistically significan（P〈0.05）.Conclusion The clinical treatment of popliteal cyst patients to meet the operation indication under the condition of the arthroscopic posteromedial approach resection of popliteal cyst surgery as the preferred way,which the traumatic are small,and can promote the postoperative recovery.
