腹腔镜下子宫腺肌瘤病灶切除术的临床分析

Clinical analysis of laparoscopic endometrioma lesionectomy

目的研究子宫腺肌病患者采用腹腔镜技术手术治疗的临床效果。方法选取2015年12月~2017年6月在我院接受手术治疗的40例子宫腺肌病患者,通过数学随机列表法分为对照组和治疗组,每组各20例。对照组患者采用腹式子宫腺肌瘤病灶切除术治疗;治疗组患者采用腹腔镜下子宫腺肌瘤病灶切除术治疗。比较两组患者手术操作时间、术中出血量、术后住院时间、手术治疗满意度、手术治疗效果、术后复发率、术后复发来院治疗次数、住院治疗所需费用。结果治疗组患者手术操作时间和术后住院时间短于对照组(P<0.05);术中出血量(32.56±5.17)ml,少于对照组(109.54±21.70)ml(P<0.05);手术治疗总满意度为95.0%,手术治疗总有效率为95.0%,均明显高于对照组(80.0%和75.0%)(P<0.05);住院治疗所需费用低于对照组(P<0.05);术后复发率、由复发而来院治疗次数少于对照组,差异均有统计学意义(P<0.05)。结论子宫腺肌病患者采用腹腔镜技术切除手术,能缩短手术操作和术后恢复时间,减少术中出血量,有效改善子宫腺肌病各项症状,使治疗效果、经济效益、患者满意度同步提高。

Objective To study the clinical effect of laparoscopic surgery in patients with adenomyosis.Methods 40 patients with adenomyosis who underwent surgeries in our hospital from December 2015 to June 2017 were enrolled.They were evenly divided into control group and treatment group by a mathematical random list.In the control group,abdominal endometrioma lesionectomy was used,while in the treatment group,laparoscopic endometrioma lesionectomy was adopted.The operation time,intraoperative blood loss,postoperative hospital stay,satisfaction on surgical treatment of adenomyosis,surgical effect,rate of recurrence after surgery,frequency of visiting hospital due to postoperative recurrence,and hospitalization expenses were compared between the two groups.Results The operation time and postoperative hospital stay in the treatment group were both shorter than those in the control group（P〈0.05）.The intraoperative blood loss was（32.56±5.17） ml,less than that in the control group（109.54±21.70） ml,（P〈0.05）.In the treatment group,the total satisfaction on surgical treatment of adenomyosis was 95.0%,and the total effectiveness rate of surgical treatment was also95.0%,much higher than those in the control group accounting for 80.0% and 75.0% respectively,which were displayed statistical significance（P〈 0.05）.The cost of hospitalization in the treatment group was lower than that of the control group（P〈0.05）.The rate of postoperative recurrence and frequency of visiting hospital due to postoperative recurrence in the treatment group were less than those in the control group,the differences were statistically significant（P〈0.05）.Conclusion Adenomyosis patients using laparoscopic surgery for removal can shorten the operation and postoperative recovery time,minimize the amount of intraoperative bleeding,and effectively ameliorate the symptoms of adenomyosis,and improve the treatment effect,economic benefits,and patient′s satisfaction simultaneously.
