内分泌科护士共情能力对住院患者抑郁情绪和NK指标的影响

Influence of Endocrinology Nurses' Empathy on Depressive Mood and NK Index of Hospitalized Patients

目的探讨内分泌科护士共情能力对住院患者抑郁情绪及NK指标的影响,为通过叙事医学干预减轻患者抑郁情绪、提高免疫功能提供初步依据。方法随机抽取重庆市某医院内分泌科30名护士,运用人际反应指针量表(IRI-C)测量共情分数,根据共情分数将其分为三组。随机抽取每位护士的12位住院患者,测量其抑郁情绪和NK指标,分析护士共情能力对患者抑郁情绪和NK指标的影响。结果患者出现抑郁情绪率为51.67%。共情分高的那组的患者抑郁情绪更轻,NK计数越高。多元线性回归分析结果显示,其责任护士共情得分、医疗支付方式、病种不同的患者抑郁情绪和NK指标有统计学意义(P<0.001)。在均衡其他条件的情况下,主管护士共情评分每增加1分,抑郁情绪评分减少0.336分(P<0.001),NK计数增加0.271(P<0.001)。结论内分泌科护士共情能力是住院患者抑郁情绪和NK指标的影响因素,基于共情的叙事医学干预应在临床实践中尝试并拓展。

Objective： To explore the influence of endocrinology nurses＇ empathy on depressive mood and NK index of hospitalized patients,and provide preliminary basis for reducing patients＇ depressive mood and improving immune function through narrative medical intervention. Methods： 30 endocrinology nurses were randomly selected from a hospital in Chongqing,and the interpersonal response index scale（IRI-C） was used to measure empathy scores. According to empathy scores,they were divided into three groups. 12 hospitalized patients were randomly selected from each of the nurses and their depressive mood and NK index were measured. Then the influence of nurses＇ empathy on patients＇ depressive mood and NK index was analyzed. Results： The proportion of patients with depressive mood was 51. 67%. The patients in the group with high empathy scores had less depressive mood and higher NK count. Multiple linear regression analysis showed the depressive mood and NK index of patients varied in their primary nurses＇ empathy score,medical payment and disease type had statistical significance（P〈 0. 001）. In the case of equilibrium of other conditions,for each additional point of the primary nurses＇ empathy scores,the patients＇ depressive mood decreased by 0. 336 point（P〈 0. 001）,and NK count increased by 0. 271（P 〈0. 001）. Conclusion：Endocrinology nurses＇ empathy is the influencing factor of patients＇ depressive mood and NK index. The narrative medical intervention based on empathy should be tried and extended in clinical practice.
